Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2021-2025 Sienna Hybrid vehicles. The bolts for the seatbacks on the third-row seat assemblies may not be tightened properly. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 202, "Head Restraints," 207, "Seating Systems," and 225, "Child Restraint Anchorage Systems."
Risk: An improperly tightened seatback may not adequately restrain the occupant during a crash, increasing their risk of injury.

The contact stated that approximately four months ago, her eldest son was entering the vehicle, he became stuck between the door and the door frame. The contact stated that on a later date while her youngest son was entering the vehicle, the close button was depressed on the outside of the vehicle, and without any mechanism to prevent premature closure, his arm became stuck between the door frame and the door while the contact was inside the residence retrieving additional items to take the children to school. The contact stated that her older daughter was able to remove him from the vehicle. The contact stated that her son sustained injuries to his arm and had marks all over his arm where it was stuck between the door and door frame. There was no medical attention provided but an ice pack was placed on the part of the arm that was stuck in the door. The contact was concerned about the safety risk posed by the failure and the lack of a sensor or mechanism to prevent the door from closing while a person was entering or leaving the vehicle. There were no warning lights illuminated during the failure. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not contacted.
