I am filing this complaint to report a serious safety defect in the Tesla Model Y. On [date], my wife suffered a rear tire blowout at freeway speeds in our Tesla Model Y. Despite regularly checking tire tread depth and relying on Tesla’s built-in tire monitoring systems, the blowout occurred because the inside edges of the rear tires had worn down to exposed cords, while the outside tread still appeared to have ample depth. Tesla Service attempted to explain this away as “normal wear and tear” and a need for alignment. However, this is misleading. The Model Y’s rear suspension has built-in negative camber that cannot be adjusted from the factory. Alignments can only adjust toe, not camber, so this design characteristic inevitably causes accelerated inside-edge wear regardless of alignment. More critically, this risk is not disclosed to consumers: It is not explained during the sales process. It is not documented in the Owner’s Manual. Tesla’s sensors only track pressure, not tread depth, meaning there is no warning system to alert the driver. The average consumer has no way of knowing they must crawl under the car to inspect the inside edges of the tires. To an owner, the tires look safe until catastrophic failure occurs. In addition to the safety hazard, this hidden defect creates a financial burden. Because of the blowout, I was forced to spend $1,200 on new tires unexpectedly. This premature and unforeseen cost compounds the danger by placing families under financial stress along with physical risk. This represents a foreseeable safety hazard that endangers Tesla drivers and passengers. I believe this condition warrants a formal NHTSA safety investigation and potential corrective action (such as a recall, suspension redesign, or mandatory disclosure). Sun glare is a well-documented driving hazard, yet Tesla’s Y model’s visors are too short to offer sufficient coverage. This compromises driver safety and forces customers to seek aftermarket solutions for a problem rooted in design. A responsible manufacturer should address this defect directly. Driving home as sun was setting, I was completely BLINDED by the sun. It was terrifying as the visor was too short to stop the sun glare. I was lucky I didn’t hit something or somebody plus my own safety.

Traveling today using the cruise control function in Tesla Model Y, the car took over the control of the car and applied braking for no clear reason. Was going 70 miles an hour, had been using the cruise control for about 30 minutes when incident occurred. There were semi trucks in the slow lane, I was in the fast lane. There were no vehicles in front of my car. The car suddenly lunged forward as the brakes were automatically applied. Simultaneously, the steering wheel began to jerk back and forth causing the car to veer back and forth. Vehicles around me began braking as well to avoid rear ending my car and to avert hitting my car as it veered back and forth. I had to fight the steering wheel in an attempt to pull it off the active roadway and then the car suddenly gave all control back to me at which time I was able to disable the cruise control and take full control.

the steering wheel used to be centered in the middle. when you turn right, its resistance would turn it left back to the center point. or when you turn left, its resistance would turn it back right towards the center point. so after each turn such as u-turn, one would grab the steering wheel lightly so it can slide back to the center position. i will call it the "no resistance" point for that centered steering wheel position. now the problem is, that no resistance point has moved a few degrees right and it brings the wheel with it. i now have to turn the steering wheel left (with the resistance that it wants to go back to right) to make the steering wheel centered and go forward straight. car goes forward straight when the steering wheel is centered so it's not an alignment problem. but in order for the steering wheel to be centered, i have to turn it slightly left because the no resistance point has shifted a few degrees right. now this is dangerous because the car can now take you off the lane easily. car is equipped with lane keeping assistance and i find the system helpful but the system is not activated some times under some circumstances. I visited a dealer and they claimed the wheel is centered that there is no need for any repairs.

shortly after i purchased this car it began to put itself into park while the car was moving slowly and even when it was in car wash mode going through a brush carwash that requires cars to be in neutral. i reached out to tesla support to investigate and they explained this is a safety feature that engages if the driver leaves the seat while the car is not in park. i explained to them that in both cases i had been sitting firmly in the seat and there should have been no reason for the car to put itself in park and asked them how i turn this auto feature off. they said there was no way to turn it off. i explained to them that ALL automation features should be optional, they argued with me. came to find out some sensor in my driver's seat was malfunctioning. they took the car to replace the seat/sensor, but i reiterated that all these auto features should be optional and a consumer should be able to turn them off. my car was rendered unsafe to drive due to the malfunctioning sensor, i should have had a way to turn off the feature, i asked them to open a ticket with their engineers to add settings to turn off this automation feature. fast forward to 6/25/25, as i am pulling my car into a parking space at a store the car SLAMS itself in park, so hard that i thought maybe i had hit something hard with one of my wheels. i followed back up with Tesla support and am being told there is still no way for me to turn off this automation feature. no automation feature should EVER be hard coded into a motor vehicle. the risk presented by malfunctioning automation features is too great, and if i want to completely control every aspect of my car i should be able to, regardless of how much automation it is capable of. i am filing a complaint now because Tesla is clearly not taking these concerns seriously

The contact stated that it was raining while driving on the highway at 45 MPH when the vehicle began to hydroplane, no warning lights were illuminated, The contact attempted to slow down their vehicle however upon depressing the brake pedal the vehicle would not stop, and the vehicle immediatley spun around 2 times before crashing into a concrete barrier with the rear and front end of their vehicle. No property damage was reported, and the vehicle came to a stop upon impact. The airbags did not deploy. The vehicle did not need to be towed. The hazard warning light was illuminated, and warning messages were displayed stating that cameras had been damaged. A police report was not filed. The vehicle was driven back to the contacts residence, and from there, towed to a local Tesla dealer where the vehicle was diagnosed and it was declared a total loss. The vehicle was destroyed. After installation, my car began displaying two error messages, and key safety features, including detection of nearby vehicles and warning systems, ceased functioning. This creates a significant safety risk when driving, as the car no longer alerts me to surrounding traffic or visualizes other vehicles on screen. Upon contacting Tesla customer service, I was instructed to schedule a service appointment, which I did for May 21, 2025, at 2:15 PM. During the appointment, the Tesla service advisor confirmed that the issue began only after the software update. However, I was told I would need to pay $3,000 out-of-pocket to repair the issue because my car is no longer under warranty. I raised the point that the malfunction was caused directly by Tesla’s software update. Despite this, I was informed that Tesla refuses to repair it at their cost, even after acknowledging the connection between the update and the malfunction. Moreover, the technician informed me that this issue could escalate and create safety hazards in 4–6 months, yet no resolution or recall has been offered. Given the acknowledgment that: The problem was introduced by Tesla's software, The malfunction affects critical safety systems, and There is imminent risk to safety, I believe Tesla should bear responsibility for the repair and ensure the vehicle is safe to operate. This situation warrants urgent attention, both from a consumer protection and vehicle safety standpoint. The steering wheel is locking up. First while parked and attempting to leave. It does eventually unlock and appears to not have what i would refer to as power steering. While driving it appears to not have power steering and the steering periodically locks up which i would immediately brake a have to repeatedly yank hard on wheel until it unlocks at this point as of the moment of this writing the steering wheel is completely locks up and cannot be yank out of lock at all. I have a video of some incidents happening. One heading into on coming traffic another of the car driving up on a snow bank ( if the bank was not there the car would have ended up in a ditch ). There are caution indicators on the screen but none say stop do not drive. The first caution or warning indicator says stability control disabled drive with caution. Next indicator states traction control disabled drive with caution. Next indicator steering assist reduced steering may require increased effort. I find myself very lucky that no real damage injury or death occurred it was that dangerous on my over an hour drive. When the steering locked up in the parking lot i did contact tesla and initially everything was fine but locked up again in parking lot. I contacted them a second time with the wheel unlocking at the but no one said dont drive nor did any indicators state that actually indicated i could drive but with caution. I believe that if i were on a busy highway things would have been much worse. I was lucky. There was no indication of a totally locked steering wheel in there diagnostics indicators also nothing stating do not drive. Actually quite the opposite. I can provide driving video upon request. There were other warning indicators i have some recorded. None of the indicators stated a totally locked steering steering wheel lock up and nothing indicating do not drive. Description for NHTSA Report: I am reporting a significant safety concern with the new Teslas (starting with the 2024 models) that do not have a traditional physical horn. Instead, the horn sound is generated through the car’s external speakers. This design poses a severe safety risk, especially in rainy conditions or when the speakers are wet, as the horn becomes inaudible or significantly less effective. Additionally, the volume of the speaker-generated horn is not comparable to the loudness of traditional horns, making it insufficient for alerting other drivers and pedestrians in emergencies. This lack of an actual horn is extremely dangerous and has already led to situations where the horn was not effective. I urge the NHTSA to investigate this issue as it compromises vehicle safety.

Since the front-facing and side-facing cameras do not work the vehicle's forward and side collision detection systems are inoperable. The vehicle suffers from additional failures (GPS navigation, Maps, Autopilot, among many others). I have presented these issues to Tesla and they have continously pushed out my repair date. I have also asked for a loaner vehicle due to the failure of the car's safety systems (some of which are federally mandated!!). Tesla has ignored my pleas for a loaner vehicle and has also remained silent when I have repeatedly asked to return the vehicle under Washington State lemon laws. My safety and the safety of others have been put at grave risk due to the backup camera and collision detection cameras not working. Tesla has NOT inspected the vehicle as they continuously push my service date further out. No obvious warning lamps, messages, or other symptoms appear.
