Software in the Approaching Vehicle Audible System (AVAS) Information Sound Box (ISB) amplifier may be missing, which can cause inaccurate, external pedestrian warning sounds. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 141, "Minimum Sound Requirements for Hybrid Electric Vehicles."

My Mazda CX90 consistently has a hard delay before jerking my car forward, especially when shifting between 2nd and 3rd gear. This is most common at slow speeds and accelerating from a stop. On top of generally rough shifting, there have been four separate instances where my car blatantly stopped accelerating and slowed my car from 70-80 mph down to 40-45 mph. Once driving up a hill, and the rest on a flat interstate. I tried flooring the gas each time and the rpms would go up to approx 6000, but the car simply would not respond. After multiple attempts to let off of the gas and re-engage the vehicle, my car suddenly began accelerating again and continued as if nothing happened. I have been able to safely move out of traffic's way each time, but this presents a great threat to myself and others as I have no control over my speed and can easily get into a collision. As if uncontrollable stopping on the freeway isn't enough, my vehicle also accelerates on its own and jolts itself forward when driving at slow speeds through parking lots and stand-still traffic. These accelerations have occurred at idle with the breaks both on and off. The vehicle only propels itself forward by a couple of feet, but the issue seems to be getting more severe each time it occurs. Luckily I have been able to control the car and stop myself from hitting others so far, but I am afraid this may cause harm to pedestrians or people in nearby vehicles. I have mentioned these issues to my local Mazda dealership on multiple occasions, but they dismissed my concerns each time and told me my vehicle is in safe operating conditions. They claim they have not been able to replicate any accelerating/ decelerating instances, and they say the car's shifting is within spec. My car has never thrown any codes or displayed warning lights on the dash. I have taken the vehicle in for all recalls and maintained it well. I recently hit 15000 miles, though these issues have been presenting since 5000.

We took it on August 29, 2025 to Colorado vehicle failed to perform loss of power going up the mountain almost got hit by a vehicle because I was in a fast lane with my family had to pull over with whatever power I had left in the engine (not the battery). I mean the vehicle just keeps failing to perform as it should as well on the way back. It had the same issues but as severe going to Colorado.Vehicle does not change gears going uphill. When we stopped in Vegas for fuel, fuel door did not open it & malfunctioned. So vehicle does not perform well in cold and hot weather AWD system does not work at all. We feel a jerking, we hear a whistling’s type of noise that the dealership fell to inspect. Per them everything worked fine but when we took it to Colorado this weekend for Labor Day, vehicle underperformed with extreme safety concerns and we contacted Mazda, but they fell to listen to our concerns and say the vehicle is fine but when you drive the vehicle, it does not perform as it should and is a safety hazard to us and anybody around us due to a loss of power and everything I described above.

While I understand that tires themselves are typically covered by the tire manufacturer’s warranty, this situation is different. Mazda equipped the CX-90 with Falken OEM tires that are inadequate for the weight and design of the vehicle, leading to premature wear at only 20,000 miles. This is not normal wear-and-tear. My claim is that Mazda bears responsibility for installing unsuitable tires on a heavy SUV, which created a safety hazard.

The rear door seals on the vehicle do not seal which allows water and dust to enter the vehicle from the rear two doors. Mazda customer service has been contacted multiple times on this issue and they refuse to address the issue. The dealer it was purchased at said it was a known issue after we had already purchased the vehicle. You can look online and see a bunch of people reporting the same issue with no resolution.

The contact stated that the message "Electrical Leakage in the Hybrid System” was displayed. Due to the failure, the vehicle was towed to a dealer where the service technician acknowledged that the failure and confirmed that the issue with the hybrid battery was a known issue. The vehicle was not diagnosed or repaired and remained with the dealer. The manufacturer was not notified of the failure. The failure mileage was approximately 24,000.

The same occurred when my elderly mother was in the vehicle. My wife called me while I was at work today to let me know that our soon to be [XXX] old toddler locked himself in the car with her keys inside while the car was running with the air conditioning on. She repeatedly tried to use the My Mazda app to unlock the car but it kept giving an error message. She called 911 and luckily he finally unlocked it on his own just prior to the police department's arrival. Upon further research this appears to be a known issue that has been brought up to Mazda from other owners and it has been dismissed by Mazda as a safety feature designed to prevent theft. Apparently this is by design, the app will not let you unloco the car if the vehicle is running with the keys in it. Personally, the theft of the vehicle is nowhere near as important as the health and safety of my child. We were extremely fortunate that the air conditioning was on as the temperatures in our town are hitting triple digits right now. What if it had not been on? This could have had a much worse outcome. Additionally, how in the world does this deter theft? So what Mazda is basically saying is that somebody would get my phone, access my password/fingerprint protected app, unlock my car from my phone, then get in and drive off. The likely hood of this happening is way lower than a curious toddler or rambunctious dog accidentally getting locked inside the running vehicle. Additionally, if my wife locked her keys in the car, and isn't home to get the spare, or im at work and can't bring her the spare right away, she's [XXX]. Braking System and Infotainment Malfunctions — 2024 Mazda CX-90 PHEV Vehicle: 2024 Mazda CX-90 PHEV Incident Dates: June 1, 2024 June 16, 2024 June 30, 2025 Ongoing (infotainment issue) Summary of Complaint: I am reporting multiple safety-related concerns with my 2024 Mazda CX-90 PHEV involving both the braking system and the infotainment/electrical system: 1. Delayed Brake Engagement – June 1, 2024: While stopping for a pedestrian crossing the street, I pressed the brake pedal and the vehicle lunged forward before the brakes fully engaged. The delay in braking caused the vehicle to move forward several inches before stopping, nearly endangering the pedestrian. It felt as if the brakes did not respond immediately when applied. 2. Sudden, Unnecessary Braking – June 16, 2024: While following a vehicle that was slowing down to make a right turn, the CX-90 PHEV abruptly and aggressively braked on its own, despite sufficient following distance and no imminent collision risk. The rapid deceleration nearly caused the vehicle behind me to rear-end us. This appeared to be triggered by the vehicle’s automated braking or forward collision system reacting too strongly. 3. Infotainment System Failure – Ongoing / June 30, 2025: The infotainment system frequently restarts itself, shutting down entirely and rebooting without warning — particularly while driving in larger cities. On June 30, 2025, the system completely failed, and a warning appeared on the dashboard stating that the vehicle needed service. After shutting down and restarting the vehicle, the message disappeared. However, this intermittent failure creates a serious distraction and potential safety hazard while driving.

Engine Malfunction have serviced. At least 3 times. One of the 3 (so called) repairs took 3 weeks. They said “Tariffs you know Japan”. When I get this warning my car engine seems to vibrate. I have only had my car since August of 2024. New vehicle I am the 1st and only owner. Today is June 22, 2025 and tomorrow will be another incident number 5. My car is doing this again. I am a [XXX]. Most of the time my car is in the garage. But I have to drive 188 miles on the turnpike to visit my son and come back. This is every 3 months or so. My other son drove to South Texas. These warnings always came on. Am I supposed to stay home and only go to the store. I pay over 700.00 a month. This is not good. I am now getting afraid to drive it. So tomorrow I will again have to drive to Edmond Oklahoma and have it repaired again. “Engine Malfunction have services”. I’m so disappointed. My son has a cx-5 and it’s a great car. I’ve had so many loaners. They have been fine. I am told this is a brand new model and we have to get the kinks out? This is at my expense, peace of mind. Please help me. Thank you. The vehicle displayed "Engine Malfunction have your vehicle inspected" on the dash along with orange lights. The vehicle would drive but without warning would not come up to speed. This caused an unsafe situation entering the interstate where the vehicle was below the minimum speed until we could exit the highway. There was no warning of the low power and inability to operate normally. This was a known problem and the vehicle had all of the recalls already performed. Dealer states there is a problem with the low pressure fuel pump. I am not confident the problem will be solved.

Hybrid battery failure. Battery issue can cause car to lock up while driving. Dealer service department indicate they have at least 6 vehicles with this same issue. The vehicle has not been inspected by other than the dealership service department that I am aware of. While driving to the dealership for a routine oil change, the car made a quick odd noise then 3 alerts came on said Hybrid system malfunction have vehicle inspected, engine malfunction, system malfunction. Dealer inspected vehicle determined needed a hybrid batter replacement, indicated they could not say vehicle was safe to drive as it could just stop working. Considering they already have a number of cars with same issues in the shop I am concerned this could be a defect problem in many vehicles.

I've noticed ever since the car was new that it had a poor sense of straight ahead especially at freeway speeds. When the lane departure chime was active it would sound constantly{has since been turned off but lighted markers in gauge cluster still fire all the time}. When I take my attention off road for a second to adjust fan speed or temp, the car is close to going into the other lane. Lots of concentration necessary. I've mentioned this to one dealer who checked and adjusted alignment with no improvement and another dealer said they've had other owners of Mazda CX-90's voice the same complaint. There's also a long term test in Motor Trend on a CX-90 that complained about the same issue. I feel there is something wrong here that could be a potential safety problem.

Steering sticks while driving. Then, all of a sudden releases, and this causes a jerk in the steering and vehicle movement in the direction of steering. Vehicle recall 6524A/Jan 18, 2024 for "increase in steering effort" was completed in April 2024. This issue is very apparent at interstate highway cruising speeds to just maintain staying in traffic lane. Worried that steering may continue sticking and not be able to maneuver vehicle to drive safely avoid an accident. Turned off the vehicle lane assist feature to diagnose the issue, the issue still exists with the lane assist feature turned off.

Bought this brand new CX90 Premium in October 2024! As of today Feb 11, the car has been in the dealership 4 x fir the same issue!! Dead battery after sitting for 2 weeks! They have replace one small battery and that worked for a month! Took it in last week for the same issue! Dead battery! We find that this is a very coming problem. They said they would now replace the 58 volt battery but it is on back order for 8 weeks. In the mean time I’m paying on this car and I have to drive a loaner which isn’t even close to my car! I’m finding out by talking to other customers this is a common problem! We didn’t get any notice or call! I am demanding this be considered “lemon law” as this is the fourth time it has been in for the same problem. I want all my money back that I paid and demand Mazda USA issue recalls for this issue. As soon as the needle dipped a fraction below 1/4 tank (about 20 miles later) the range indicator dropped instantly to 40 miles left AND THEN DROPPED ONE MILE PER SECOND UNTIL IT REGISTERED ZERO. The needle also fell at the same rate until I was driving on the interstate with zero range and a gas gauge buried below empty. All of this occurred within one minute. I pulled into the nearest gas station and filled up with 12.71 gallons (it's an 18.5 gallon tank). So I actually had 5.79 gallons left (or 156.33 miles of range at 27mpg on the highway). Not being able to rely on the gas gauge/range indicator is a safety issue.
