Last time I changed my oil I had bronze colored flakes of metal in my oil. It looked like parts of the main connecting rod bearing material. No running problems YET, but will likely fail soon. I've kept up on oil changes every 5-7K. 89K on vehicle. Please do something about a full recall on this issue and include this report into the affected VINs.

I am the original owner of my Honda Pilot and have always taken the vehicle to Honda dealerships for all scheduled maintenance and service. On [XXX], I noticed the air conditioning was blowing only warm air, with no cold air at all. I scheduled an appointment at Germain Honda in Beavercreek, Ohio, where the vehicle was inspected on July 28, 2025. I was informed that the A/C system had suffered what is often called “A/C black death” and that the entire A/C system would need replacement at a cost of $6,753.93. I was also told that no parts were available and that I would be contacted when they became available. I have not been contacted since. When I asked why this happened, I was told it was rare and that I was just unlucky. However, my own research has shown that many Honda Pilot owners (and likely Ridgeline owners as well) have experienced the same failure, sometimes repeatedly, even after dealer repairs. Honda has issued recalls and extended warranties for A/C failures in other models such as the Civic and CR-V, but not for the Pilot, despite widespread reports. This is a serious safety issue. Where I live, July and August were extremely hot. The lack of A/C made the vehicle unsafe to operate, as every year children and pets tragically die in overheated vehicles. Driving without functional A/C in extreme heat creates a health hazard for families. I was unable to use my vehicle at all during these peak summer months. My Pilot has fewer than 100,000 miles, has been well maintained, and should not have suffered a complete A/C system failure this early in its life. Honda has clearly known about systemic A/C issues in multiple models yet has not taken responsibility for Pilot owners. I own a 2020 Honda Pilot EX with approximately 58,000 miles. Every time I drive on two-lane roads, I receive a bright orange “Brake” warning from the Automatic Emergency Braking system when vehicles in the opposite lane are passing by. The system falsely detects an imminent collision, even though the oncoming vehicles are safely in their lane. The system then applies the brakes unnecessarily. Most of the time it brakes mildly, but on at least two occasions it has braked very hard, causing the vehicle behind me to nearly rear-end my car. This creates a significant and dangerous situation, as it could easily cause an accident. I now have to disable the automatic braking system every time I drive. During my last drive, after disabling the system, I received a warning message stating: “ABS Failure — Take to Dealer.” I have also experienced dangerous behavior from the Lane Departure Warning/Assist system. When exiting the freeway, if the lane lines disappear or become unclear, the vehicle has slammed on the brakes suddenly. On one occasion, this caused me severe whiplash. Thankfully, I was alone in the car — if my young children had been passengers, they could have suffered serious neck injuries. I have taken the vehicle to the Honda dealer and independent mechanics. Each time, they have only cleared the codes and told me it was due to “dirty sensors.” The warnings and braking issues, however, keep coming back. This issue is persistent, reproducible, and unsafe. The Automatic Emergency Braking, Lane Departure, and ABS systems are not functioning properly and create a real danger to myself, my passengers, and other drivers on the road.

I'm concerned these issues may be related to ODI investigation PE25008 and further use of my vehicle without having it inspected may result in a crash, fire and/or complete engine failure. The idle engine turn back on feature has not turned on several times. It has happened a couple of times upon turning it back on after a complete turn off in a parking lot. Also, it has happened in traffic at a stop light on busy roads where I was sitting there dead stopped in the road unable to get the car started. Another time, it did not turn back on as I was pulling out of my neighborhood and we almost got hit because of it. The battery seems to be involved and seems to have to recharge for a few minutes and then it will turn back on. I've called 911 to help me get out of the road, have taken out the battery and put it back, and had it checked at the dealership. I have to remember to use the button that turns off the idle engine turn off feature or it will happen. It doesn't have any pattern or reason to it.

What component or system failed or malfunctioned, and is it available for inspection upon request? When calling the dealer they believe it is the “Smart Control unit.” How was your safety or the safety of others put at risk? My vehicle will not start most of the time…leaving me stranded when I have to go anywhere and I have to get out to take the battery terminal off and back on just to try to reset it long enough for it to try to start. Has the problem been reproduced or confirmed by a dealer or independent service center? No, from my understanding of reading other people having the same problem Honda knows it is a common issue and has not done anything about it. The dealer wants paid to do an inspection. I do not have the money they are asking for. I know inspections aren’t very expensive but most people (myself included) can’t afford even a simple inspection. Just to pay for the car takes all I have. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? At first I noticed my car alarm would go off randomly and originally thought my doors weren’t being shut all the way however after making sure all doors were in fact shut it still went off. I was at work that night and had to keep going out to turn my alarm off…the next morning I go in to start my car and leave and it had trouble starting saying “keyless start system problem” after finally getting it started I drove to the gas station to get gas and turned off the engine as I was walking away I heard cracking noises and my car making the ding noise it makes when you turn it on ACC mode. When I came back to the car all the lights on the dash, radio screen, headlights, tail lights, and mirror lights (the ones that light up when a car is close) were flashing like crazy and still heard the crackling noise.

I am writing to formally report a safety concern related to a recent maintenance issue on my Honda vehicle. Approximately one month ago, I had my vehicle serviced to address an issue with the rear tailgate lighting. Although the issue was supposedly repaired, I discovered during nighttime driving that the tailgate lights were still not functioning properly. Despite the headlights being on, the rear tailgate lighting failed to illuminate, creating a serious safety hazard. I was nearly involved in a rear-end collision with a truck due to this malfunction. Upon stopping and inspecting the vehicle, I confirmed that the tailgate lights were not working as intended. This problem raises serious concerns, especially considering the dealership performed a multi-point inspection at the time of service. The malfunction was not detected or corrected adequately. Additionally, I question the reliability of these inspections, as there have been inconsistencies in their findings. For example, one inspection indicated that my brakes were fine, but just three weeks later, another inspection claimed they required replacement. The main issue, however, is that the tailgate lights were supposedly repaired but were not correctly configured to turn on automatically with the headlights. This oversight directly jeopardized my safety and the safety of others on the road. I respectfully request that NHTSA investigate this matter and determine whether there is a broader issue with the repair or inspection protocols being followed by Honda service centers, particularly regarding rear lighting systems and multi-point inspections. My 2020 Honda Pilot has 53k miles on it. I have have the vehicle for 8ths and 8k miles. The vehicle engine light suddenly began flashing and the vehicle began running rough. A loud knocking sound was coming from the engine when accelerating. The vehicle was taken to a Honda dealer and analyzed. The testing showed that the camshaft and cylinder #2 rod bearing was damaged to the point of the engine needing replacement. Upon research of the problem and damage found by the dealer, it matches a major issue found on many Honda's and Accura's. A manufacturing defect in the crankshaft leads to excessive wear and failure of the connecting rod bearings. This problem can cause the engine to suddenly seize or fail. Honda is currently refusing to cover the repair cost because the VIN of my vehicle isn't on the recall list.

The “smart” control module, part# 38320-TG7-L112-M1 was removed and inspected; obvious incomplete conformation coverage was discovered along with significant dendritic growth on an uncovered IC. Proper cleaning of the board alleviated the issue – for the time being at any rate. I am including pictures of the board and growth that clearly show this defect and its related consequences, but I can provide the part for inspection if it’s feasible timewise and if I haven’t sold the vehicle by that point. •How was your safety or the safety of others put at risk? The vehicle was rendered completely and utterly inoperable by this condition and additionally necessitated the removal of the battery to keep the car alarm from triggering randomly. If this condition manifests itself at the wrong time it could easily create a dangerous situation for myself, my family and others. •Has the problem been reproduced or confirmed by a dealer or independent service center? The diagnosis, research, documentation and associated repair were performed by an independent service technician. •Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? This problem was reported to Honda USA in late March of 2025; no acceptable recourse was provided or offered. •Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No. On Friday March 21st at approx. 6PM, the car alarm was triggered with no interaction whatsoever after the vehicle had sat for several hours. After disconnecting the battery for over an hour the problem immediately returned upon reconnection along with flashing dash & interior lights and clicking relays and/or components.

[XXX]: While driving on [XXX] in Pennsylvania, my check engine light illuminated in a tight construction zone with jersey barriers & speed limit of 55 mph during inclement weather. I chose to remain behind a tractor trailer driving under the speed limit for the remainder of the construction zone approximately 1 mile. Once out of the construction zone, I attempted to pass the truck in front of me; however, when I moved into the passing lane, my vehicle would not accelerate to pass the truck placing myself and my 2 children in danger on [XXX] in inclement weather. Fortunately, I was able to safely return to the travel lane & then exited at the next exit which was about 2 miles away. I proceeded to a pull off right after the exit ramp & called my spouse. When he arrived, we decided to attempt to drive the vehicle home an alternate route; however, while driving about 2-3 miles the check engine light remained on & I was not able to accelerate beyond 35 mph. We decided to pull into our HS parking lot where I shut off my car for about 5 minutes. When the vehicle was restarted, the check engine light was no longer on, so we drove around the parking lot & the vehicle appeared to be operating normally allowing acceleration to occur, so we attempted to return home. While driving about 3 miles, the check engine light once again displayed as well as a series of other warnings. The vehicle began to shake and various noises were produced from the front end of the vehicle including what sounded like metal on metal grinding. ASAP, I proceeded to a parking lot to ensure safety for myself & children to stop the car and have it towed to a local mechanic, then to a Honda Dealership. After initial diagnostics, we were informed our engine failed & needed replaced due to what they suspected was a connecting rod bearing issue. I was traveling on the highway in Norwalk, CT and the dashboard lite up with the same errors and he brakes did the same thing and the car was hard to steering. The car was never inspected before it was sold to me. I also never received several documents from the dealership required by CT state law. I left the car. The sun roof shattered without provocation while driving in the interstate. Glass showered in the cabin to the driver, front passenger and floor in the back seat. Temp outside was between 55-57F. Inside cabin heat in system was at 69F. Seat warmer were on. It was raining at the time of incident between 2:30-3:15pm. I (driver) was very shaken by incident but did not lost control of steering. Initially could not correlate explosion like sound to location. Thought it was in rear of car but upon feeling rain and recognition of glass in the cabin realized it was the sunroof. Vehicle was at around 60-65mph in 70mph. This is memorable as GPS send notification of speed checks on zone prior to incident. Car in front was very far ( more than 10 car lengths) in two lane road. Car in rear was also at about 2 car lengths. Not reported to insurance as deductible is higher than cost of repair. Did not reported to police as no other vehicle involved and no loss of vehicle control. Definitely a safety risk. Both passengers were wearing glasses, and long sleeves due to weather and glass was all over driver and Passeger. Explosion sound was terrifying and unsettling. No warning lights or sounds noted prior to the incident. Car had been driven the day before. Car is always stored in garage. This was first drive of day for car.

This vehicle has shown juddering and difficulty with shifting gears, especially on an incline, although it RPMs will speed up and slow down even while traveling on a flat road at a consistent speed. The Honda dealership stated that the course of action is to drain and fill the transmission 3 times to the tune of $550+ and Honda Corporate claims this is "Routine Maintenance", which it is not. I have an extended warranty which would cover the torque converter but will not cover this fluid. As many other posts have indicated, this is not an issue limited to the 2016-2019 Pilots, but that it extends to 2020s as well. Continuing to escalate this with Honda Corporate, and will pull in my state's Attorney General for these unfair practices of lack of warranty coverage for a known issue that is due to manufacturing.

My issue is with existing recalls of the following: 23V-431 & 23V-858 in that replacement parts are NOT readily available according to multiple phone calls to a local dealership. The dealership responded by placing one on a waiting call list. I’ve owned this Honda Pilot over a year and have FIVE such notifications from Honda America. It is like they prefer I sell it or wreck it to get from under all these notices. Does NHTSA have legal authority to fine or demand Honda America conform to build replacement parts in straight forward i.e., as soon as possible. The vehicle surges up and back when climbing hills or attempting to pass. It is very disconcerting at the least or dangerous at worst. I will appreciate any help NHTSA can provide to get more fuel pumps with proper impellers out quickly.
