FORWARD COLLISION AVOIDANCE: WARNINGS

While traveling at approximately 55 mph yesterday, in daylight, with no other cars in front, back, or to the side of our car, it radically braked on its own. The collision mitigation system warning light has frequently flashed on the dashboard since we bought the car new in 2019 but never did it apply the brakes on its own. Dealerships over the years have told us "this just happens", and offered no fix for the false positive alerts. This is a dangerous situation which could result in an accident or even death of occupants if not resolved by Honda. I spoke with several dealerships since the phantom braking event and they tell me they hear this complaint often. Being an intermittent situation, it is extremely difficult to diagnose and resolve. Something needs to be done.

ENGINE

My family's 2019 Honda Accord with ~80k miles suddenly had all the warnings lights flashing on the dashboard in early July 2025. The initial diagnostic reader showed a Cylinder 3 misfire. After replacing spark plugs, the warning lights returned after one week; at that time, the ignition coil was replaced but the warning lights returned again after one week of driving. The Honda dealership performed further diagnostics and tests, ultimately reporting that the engine needed fuel injectors replaced AND a head gasket. It is important to mention that when we initially called the dealership's service department to make an appointment, the Service representative that answered the phone immediately recognized this issue, saying that it is a well known issue with Accord 1.5 turbo engines. Honda Corporate has stated that the car is outside of the manufacturer's warranty and that there is no recall. Note that there are class action lawsuits in the US on this matter, as well as lots of online traffic of car owners experiencing this issue with the Honda 1.5T engine. It appears that there is a serious design flaw in the engine.

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

Driving along at approximately 41 mph, CMBS unexpectedly activated for no apparent reason. There was no object in road to potentially collide with. Warning system barely activated before CMBS engaged braking very hard. Vehicle went from 41 mph to 31 mph in 2 seconds as recorded on dash cam. This is the 2nd occurrence in the same approximate location, both for no apparent reason & with no object in road ahead. Had an another car been following closely, this would have caused injury.

ENGINE

While driving my 2019 Honda Accord, the engine began misfiring and losing power. A diagnostic scan showed misfire codes (P0300–P030X). The issue was traced to coolant leaking into the combustion cylinder due to a head gasket defect. This caused engine hesitation, reduced acceleration, and risk of stalling while in traffic. I have learned that Honda has issued a Technical Service Bulletin (TSB) for this issue, but no recall. This is a serious safety concern because loss of power or stalling could cause an accident, especially at highway speeds or during turns/intersections. The repair is extremely costly for owners, and the defect appears to be widespread in 2018–2019 Accord models with the 1.5L turbo engine. This should be investigated and addressed as a recall to protect drivers.

On May 13, 2025, I took my car to the Honda dealership, because all of the lights on the dashboard came on. I was told that the clips on my turbocharger needed to be replaced, and so Honda fixed that $120 (diagnostic fee) $209.99 to fix the issue. I was traveling so they kept the car for a week. I received the car back, and about 2 weeks later, 06/03, the same thing occurred, I was told that the engine air filter needed to be changed, and it caused the lights to go off $32.84. The next day 06/06, the lights went off again. I took my car back to the Honda dealership and they had the car from 06/09 - 09/04. Today they have reached out to say “It took a while to recreate the issue because they are ‘masking each other.’ There is a leak in the head gasket, and the turbocharger is going out.” It would be cheaper to buy a used engine and used turbocharger than to have it rebuilt.” I have spoken with other Honda owners, who have a 2018-2022 Honda accord, that have experience the same issue.

FUEL/PROPULSION SYSTEM

Although my VIN currently shows no open recalls (because the fuel pump recall was previously marked completed), the replacement part failed while driving in traffic, leaving me stranded. This vehicle previously had the low-pressure fuel pump replaced under the Honda recall campaign. On [XXX], the replacement fuel pump failed again while driving in rush hour traffic in North Dallas, causing the vehicle to stall in a live lane and creating a major safety hazard. I attempted dozens of restarts without success. The vehicle was towed to Coit Auto Center ($80), where they performed a diagnostic test confirming low-pressure fuel pump failure codes — the same component that was recalled. On August 16, 2025, the vehicle was then towed ($223) to John Eagle Honda in Dallas (the original selling dealer and recall repairer). Honda and the dealership stated the repeat failure was not warrantable because the recall repair had already been performed once. This is unacceptable. A recall remedy that fails is still a recall compliance issue, not a customer-pay repair. I am requesting that NHTSA investigate Honda’s handling of repeat recall failures. The vehicle requires replacement of the defective pump and coverage of labor costs, as well as a full diagnostic inspection (starter, alternator, battery, ignition, ECU, and electronics) to ensure it is safe for road use after the stall and restart attempts. 2019 Honda Accord 1.5L turbo has confirmed cracked head gasket at 94,000 miles verified by certified mechanic. No prior warning light indicator. Car began misfiring and jerking while driving and was unable to accelerate. Took several weeks before the dash displayed any warning light indicator. After doing research, we were made aware there is a class action lawsuit against Honda Motor Company for this being a known issue in 2018-2022 Accords and it was never disclosed or addressed properly by Honda. This is a major safety concern as the car can shut off while driving, jerk, shake and lack acceleration.

My car has been sending several warnings including: Brake System: Electric Parking Brake System Problem, Brake System Problem, Brake Hold System Problem, Adaptive Cruise Control System Problem, Collision Mitigation Braking System Problem, Road Departure Mitigation System Problem, Supplemental Restraint System Problem, and Power Steering Reduction Problem. My car is not even 100,000 miles yet. I have had all maintenance done on time and recently had brakes replaced. I have changed my battery and tires twice within the 6 years of owning the car. It seems this is a regular complaint for such low mileage. I was a Toyota person but switched to Honda for their safety performance, and now I am regretting that. I read that this is the Turbo needing to be changed. This part should be recalled if it is failing soon. A person who just bought a car should not be paying $4500 in repairs already. I had the Fuel Pump recall replacement done, and had the brakes done. I am changing the spark plugs and ignition coils next week. Honda needs to do something about this and I am looking to join in the class action suit against them, if they do not correct the issue.

ENGINE

Component or System Failed: The head gasket appears to have failed, leading to coolant leaking into cylinder 4. This has been confirmed by two separate service centers. The vehicle is currently available for inspection if needed. Safety Risk: The failure caused engine misfires and dashboard warning lights to activate while driving, which could have led to a sudden loss of power or control. I had to pull over and restart the vehicle to regain drivability, which put my safety and that of others at risk. Reproduced or Confirmed by a Dealer or Service Center: Yes. The issue was first diagnosed by Honda of Sumner, who initially suspected a compression issue. Their master mechanic later confirmed it was likely a head gasket failure. A second opinion from Greg’s Japanese Auto confirmed the diagnosis and provided photos showing coolant in the cylinder. Inspected by Manufacturer or Others: Yes. The vehicle was inspected by Honda of Sumner and Greg’s Japanese Auto. No inspection has been done by police or insurance representatives. Warning Lamps or Symptoms Before Failure: Yes. After a recent service (which included an AC repair, fuel pump recall, and oil change), I experienced two misfires in cylinder 4 within 24 hours. All dashboard warning lights came on during one of the incidents. These symptoms began four days after the service. Additional Context: I was quoted $8,100 for the head gasket repair by Honda. I believe this issue could have been detected earlier during the diagnostic service. I’ve since learned of a class action lawsuit involving this exact issue in my vehicle’s generation, which makes me concerned this is a known defect.

I bought this 2019 Honda Accord on June 30th from a company called Greenville Motor Company. July 1st, I took the car to the Honda dealership to get a diagnostic to make sure everything was in good shape. The technician I spoke with did notice the car & stated that it had a problem with the previous owner which was the Injectors. She told me that the light may not come on now but it may come on in the future. After I left the Honda dealership, I went to Greenville Motor Company & showed them the diagnosis I had done at Honda. He said he didn't see anything wrong. On July 2nd, all of my dashboard lights came on. Took it back to Greenville Motor Company, the tech there said I could pick it up July 3rd & drive it over the weekend. (basically he reset the lights so they wouldn't come on for a certain period of time).Within this time period I took it back to the Honda dealer & they told me that I also needed a turbo, they gave me the printout for that as well. Took it to Greenville Motor Company, they agreed that I did. need a turbo. Manager says he ordered turbo. I I don't remember the date I took it back but on July 12th, my dash lights came on again. I took it back to Greenville Motor Company July 14th, July 18th Greenville Motor Company called & said something on top of the turbo wasn't screwed on thats what was triggering the turbo code to show, so he never put on the turbo that he ordered I had it 2 hours & my dash lights came on again.

Took my accord into service due to engine light warning, vibration on start up and to get the recall fixed. Was informed that my head gasket was misfiring and needed to be replaced, and was quoted $4515.37. The vehicle had been well maintained and low mileage at 72,000. I decided to trade my vehicle in for a new one, but received $3500 less on trade in due to the bad head gasket. I'm also concerned about the safety hazard this could potentially have been. My daughter has a 2020 accord.

NHTSA Vehicle Safety Complaint Vehicle: 2019 Honda Accord 1.5T Engine: 1.5-liter Turbocharged (L15B7) Mileage at Failure: 88,000 miles Issue: Blown Head Gasket / Coolant Leak / Engine Stalling Summary of the Problem: At approximately 88,000 miles, my 2019 Honda Accord 1.5T began exhibiting signs of a coolant leak and engine performance issues. After diagnosis by a certified mechanic, it was confirmed that the head gasket had failed, causing coolant to leak into the engine. This led to overheating and a dangerous engine stall while I was in active traffic, posing a serious safety risk. Safety Concern: The vehicle stalled unexpectedly in traffic due to the head gasket failure, putting me and surrounding drivers at risk of a crash. A blown head gasket can cause sudden overheating, misfires, and loss of power, all of which jeopardize the safe operation of the vehicle. This occurred without any warning lights or prior symptoms, making it even more dangerous. Repair Cost: The repair for the blown head gasket and associated engine work cost me $3,800 out of pocket, despite the vehicle being well maintained and relatively low mileage for this type of failure. Maintenance & Vehicle History: The car was serviced regularly according to Honda’s recommended schedule. There were no prior signs of overheating or misuse. The stall happened suddenly, requiring the vehicle to be pushed to safety to avoid blocking traffic. Wider Problem Indication: This issue appears to be widespread among 2019–2021 Honda Accords with the 1.5T engine. Many owners report similar head gasket failures, indicating a potential defect in the engine’s design, materials, or thermal management. Requested Action: I respectfully request that the NHTSA investigate this issue as a safety-related defect, and that Honda be required to issue a recall, extended warranty, or reimbursement program to address these premature and dangerous failures.

In December 2024, all of the lights on my dashboard came on & my car would not accelerate. The code indicated that it was a "Cylinder 2 Misfire." I replaced the ignition coil for cylinder 2 but my car continued to have the same issues. I had all 4 ignition coils replaced and all 4 spark plugs. In March 2025, all of the lights on the dashboard came on again with the car not accelerating properly. I had it looked at again, was told that it was a "Cylinder 2 Misfire" again but all 4 ignition coils were working properly. A spark plug was replaced at that time. Then, in May 2025, the car did the same thing with all of the lights coming and the car would not properly accelerate. The car again read that there was a misfire with "Cylinder 3" but all of the ignition coils worked properly. I had the fuel injectors replaced and another spark plug replaced. The car didn't even make it 24 hours before all the lights came on again. The RPMs in the car now go past "3" but will not accelerate & shift gears. The Honda Dealership has advised me that the head gasket needs replaced. My car has 74,000 miles on it. I had it inspected April 2024 with no head gasket issues & the car was at 64,000 miles approximately. So, in 8 months' time, the head gasket was blown & the car is well under 100,000 miles. I have been told that the engine my car has, 1.5L turbo with Honda has had this issue to the extreme that a class action lawsuit has been filed as Honda refuses to do any time of recall or assist with repairs. I literally had to drive with my 4 ways on to avoid being hit in an intersection due to the inability of my car to accelerate. The NHTSA needs to investigate Honda & determine if they are defrauding consumers by refusing to issue a recall due to this common & dangerous issue. I have owned 7 hours in the 30 years I have had a driver's license & never once had a head gasket issue or any issues to this extreme. I have never replaced a head gasket in any car that I have owned.

FUEL/PROPULSION SYSTEM

I had a recall on my fuel pump impeller and Honda replaced the pump AFTER having me wait over 1 year from me beginning to see problems with the car because "they did not have enough parts to go around". After finally receiving a letter stating I could bring the car in to have it replaced I took it to a dealership and had it replaced, by that point, I had experienced the vehicle shutting off while I was driving and having difficulty starting. After they replaced the fuel pump they advised me that the injectors were bad and needed to be replaced. The manufactors knows the part is bad, which is why they discontinued this part and created a new one to replace it, however they're getting out of helping anybody with the cost because they're not recalling them- a simple google search will show thousands of people experiencing this same issue and Honda not taking accountability. The lights on the dash first appeared about a year and a half ago, I called Honda about my recall and they told me they will not replace the part and that it is unrelated (keep in mind this was a phone call- the man had never even seen my car and he told me with certainty this was not related to the recall). The reason I keep mentioning the recall on the pump is because the fuel pump sends the fuel to the injectors, if the injectors are not getting enough, or even getting too much fuel sent to them this will cause them to fail- I believe that due to the faulty pump it caused the injectors to go bad and because Honda made everybody wait to have the recall replaced they should also be recalling the injectors that are also bad and faulty, further support- why would they discontinue a perfectly good part? Because they know the injectors are bad and they're trying to get ahead of this. I opened a good will case with them and they denied it, I'm very hopeful that you will do better research than they did into the circumstances and see the story without bias.

My 2019 Honda Accord with the 1.5L turbocharged engine is experiencing serious engine issues, including coolant leaking into the engine cylinder, oil contamination, engine misfiring, and overheating. Honda has not issued a recall or repair program for this defect, and I am now out of warranty and facing costly repairs. This is a safety concern and I urge the NHTSA to investigate.

While driving my 2019 Honda Accord at approximately 65 mph on the highway, the car suddenly lost power, went into limp mode, and the steering wheel locked up. I was unable to turn or control the vehicle, which resulted in another car crashing into me. I sustained injuries in the accident and required medical attention. The incident was extremely dangerous and could have been fatal. After being towed to Russell & Smith Honda, they confirmed a blown head gasket was the cause of the failure. This issue is already the subject of a class-action lawsuit due to known design flaws in the 1.5T engine. Honda only offered partial assistance, leaving me responsible for thousands in repair costs. This vehicle is unsafe and needs to be investigated immediately.

Vehicle Information: Make: Honda Model: 2019 Accord Sport VIN: [XXX] Mileage: 90000 Date of Initial Recall repair under service bulletin 19-091: 6/02/2023 Issue Description: Type of Complaint: Non-safety related but performance-affecting system failure (A/C system). Description of the Problem: In 2023, the A/C system in my 2019 Honda Accord Sport began to leak refrigerant due to a faulty A/C condenser and O-rings. The vehicle was repaired under Honda service bulletins (S/B 19-091, S/B 21-014–21-018), which replaced the condenser and O-rings. Within two years of this repair, the same issue has occurred again: refrigerant is leaking from the A/C system, leading to a failure of the system to cool properly. Despite the initial repair being done under warranty, I am being quoted for additional repairs, which suggests a systemic issue with the A/C system. There is no recall issued for this problem, but the fact that the issue has recurred so soon after a factory-approved repair raises concerns about the durability and quality of the parts involved. Safety Impact (If applicable): While this A/C failure is not immediately safety-critical, a malfunctioning A/C system could lead to driver and passenger discomfort during extreme weather, potentially distracting the driver and compromising the vehicle’s overall comfort and performance. We were sitting at a red light and when accelerating once green a warning appeared on the dash stating the EVAP system was failing and to see dealer. Then more warning appeared stating the following systems were failing, ABS, TPMS, Cruise Control, Driver Assist, Electronic Power Steering. Basically all electronic systems were failing. The car would not accelerate properly. The car would not go over 2500 RPMs making acceleration nearly impossible and the car would lose speed when traveling up a hill. We managed to make it to a school parking lot to safely pull off the road. The car had to be towed to a local mechanic. This all happened with zero warning.

1. Headgasket, fuel injectors, and fuel pump failed on this vehicle. Upon inspection Gwinnett Place Honda responded that this was a very common problem and that they see upwards of hundreds of cars per month equipped with the 1.5 liter turbo charged engine. 2. This affected my safety because during the whole ordeal my car went into a “limp mode” while doing 80mph down a busy highway causing me to rapidly lose speed and most cause a major accident. Unable to get off the road way due to my car shutting off I needed to have bystanders block a lane and push the vehicle off. 3.Yes it has been reproduced by both a Honda dealership and an independent service center 4.Honda stated that they wanted to swap the motor so they could send it to the manufacturer to have it studied, however in return I would be getting a motor with 90,000 miles while having to pay $12,000.00 out of pocket. 5.The car was showing a “Christmas tree” dash with every light on ranging from check engine, power steering, abs, lane mitigation, and climate control. On early mornings the car would have a misfire on startup due to the fuel injectors and during the day it would lose coolant however NOT leak it out.

I was terrified when this happened as I was traveling 70 miles per hour on the interstate and didn't know what was happening, I was unsure what was happening and if I was going to be able to control the vehicle at all even through the gas pedal, or brakes. I was terrified I wouldn't be able to steer or brake or if the car was going to accelerate without my control. I was able to brake slowly and pull over to an exit ramp. Driving it into town with no power steering was extremely difficult and scary. I had never experienced any issued with any electrical, steering, or any other systems in my car prior to this. ENGINE

All of a sudden I was driving and every light on the dash lit up the car starting stalling and shut off We replaced the spark plugs and the coils because they told us that was the problem and it's still doing the same thing quite a few people that are having this issue got a recall letter.. I never received a recall for this problem And every time I put my VIN number in it's saying zero recalls something is wrong this is not safe for me and my children
