On [XXX], I was driving westbound on [XXX], between Kerrville and Junction, Texas. I noticed a diesel fuel smell and pulled off the highway to investigate the source. Upon stopping, fire erupted from the engine compartment and quickly spread to the front dashboard and front passenger seating area. No warning lamps, no high temp or abnormal gage levels. The Mountain Home, TX volunteer fire dept arrived about 30 minutes after we called 911 and extinguished the remaining flames. They commented that a cracked / failed secondary fuel filter housing could be to blame. My insurer, State Farm, under claim #[XXX], now has possession of the vehicle remains. I told State Farm that NHTSA opened an investigation into possible fuel filter housing failures / fires. I do not know what their intentions are with the vehicle remains. The vehicle was not diagnosed or repaired. The manufacturer was informed of the failure and a case was filed. The manufacturer referred the contact to the NHTSA Hotline to report the failure. The failure mileage was approximately 212,000.

The contact owns a 2016 Ford F-250. The contact stated while driving at an undisclosed speed, the vehicle experienced reduced power and poor fuel efficiency. The messages "DPF System Overloaded" and "Exhaust System Fault - 50 Miles Until Speed Restrict" were displayed. The vehicle was taken to the dealer on four separate occasions. The dealer performed a manual regeneration three times, but the failure persisted. The vehicle was taken back to the dealer, where it was determined that the Diesel Particulate Filter (DPF) system needed to be replaced. The vehicle was not repaired. We called our local Ford dealership, Matthew Ford of Pryor (Oklahoma) and made an appointment for the safety recall of replacing the clock spring. They proceeded to attempt the replacement and in the process damaged the steering column module, part no. GC3Z3F791KA. We are now being told that Ford does not have the necessary steering column module part and nobody manufactures it for our trim level, Platinum. Consequently, our truck is not able to be fixed and returned to us in the same condition it was given to the dealership. It is still at the dealership, in pieces. We dropped it off on March 27th, 2025.

Ford has issued a number of recalls for issues on Ford F-Super Duty (F-250) with rearview cameras in their vehicles. I am also experiencing the same issues with my 2016 Ford F-Super Duty (F-250) rearview camera that displays a blank or distorted image causing no visibility while backing up, increasing my risk of a crash. The issue, Covington Ford Inc., Service Department located at 9101 Dr Martin Luther King Ave, Covington, GA 30014 (770-786-3432) explained to me is that it is caused by a loss of electrical conductivity due to fretting corrosion on the terminal header connector inside the camera and misalignment of the connectors. I am also experiencing a full black image on the SYNC screen when placed in reverse or when the 360-degree view is selected as well. Diagnostics were completed 5/21/2025 costing me $150.00. Proof of diagnostics cost can be provided upon request. Covington Ford Inc. indicated I would need a new camera installed at a charge of $727.24 + tax not including the cost of the diagnostics. Estimated total cost of repairs including diagnostic charge would be $877.24 + tax. In addition to being a safety hazard, I use this vehicle to back up to and connect to trailers necessary for my income and need to have this repair completed.
