FUEL/PROPULSION SYSTEM

The vehicle a 2024 F150 Hybrid with under 3,000 miles. First reported engine warning light was on 4/4/2025 at 2:23PM. The warning light indicated that "service engine soon alert is on because an issue has been detected with the fuel tank or lines" After a week while waiting for the appointment date the warning light went off. Then on 4/24/2025 at 3:28PM the same warning light reappeared. Took the truck to the dealership and they said they had to drop the. fuel tank to repair and issue. After 2 days the vehicle was returned to me. The problem was as noted on the Ford dealership invoice states, "Fuel Vapor Vent Valve is leaking". That was replaced. Also the initial diagnostic noted the "FTP would not hold vacuum even when canister and vapor blocking valve are closed"

LANE DEPARTURE: WARNING

The weather was heavy rain. The road was a two lane rural roadway with a 35mph speed limit. There were no other vehicles present and there were no obstructions in the roadway. As I slowed approaching a stop sign, the collision warning system activated. Traction control activated. The vehicle began braking. The transmission put itself in Park at ~15 mph. When the vehicle stopped moving it turned itself off. The vehicle came to a stop in the lane it was traveling in at the time it began to malfunction. The vehicle transmission would not come out of park. When I attempted to restart the vehicle it activated the horn until it was turned off again. Safety issue was the vehicle sudden stop and coming to rest in the travelled portion of the roadway. The vehicle was towed to Battlefield Ford in Culpeper VA on the date of the incident ([XXX]) and it was returned to me on [XXX]. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

SERVICE BRAKES

What component or system failed or malfunctioned, and is it available for inspection upon request? When the brakes were applied, the vehicle accelerated instead of slowing down/stopping. The vehicle is available for inspection and we are begging for someone to investigate this as we do not want to get behind the wheel of this truck if the issue is determined and rectified. How was your safety or the safety of others put at risk? Because of the above issue, the car accelerated and crashed into a building putting the driver and passenger at risk. The airbags deployed. The vehicle went over a walkway where, luckily, no one was walking or they would have been seriously injured or potentially killed. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Ford keeps telling us to work with our insurance company. The insurance company did an estimate for the repairs, but they are not able to inspect the vehicle and determine the cause of the malfunction. Our insurance adjuster said that Ford needs to be the one to access the black box and perform testing to determine the malfunction. Ford has been unresponsive to our claim and the service manager where the vehicle is sitting will not touch the truck due to it being a Ford liability. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No, the vehicle just suddenly accelerated when the brake was applied. The airbags deployed when the truck hit the building, the horn went off and wouldn't stop, and the brakes are stuck (unable to apply pressure).
