I recently purchased a 2023 chrysler pacifica hybrid that did not have a recall listed for the loss of power incident. Within 24 hours of buying the car it died on me while driving. I was driving on a two lane busy road when without warning the vehicle beeped loudly and suddenly I lost all ability to use propulsion. The car illuminated that I should brake and it will shut off soon but soon was immediate and I had zero time to safely exit the road. After dying a little turtle symbol showed up on the dash and the check engine light became illuminated. Once I was able to get the vehicle restarted the check engine light remained illuminated and ai arranged for the car to be seen at the dealer right away. I took it to the dealer within a few minutes of this event and they did the update that was supposed to fix something. Within 3 days it repeated the same event. It is now sitting at the dealership to be looked at again. I had no time to stop safely, had I been on the highway the ability to remove myself safely from the road would have been zero percent chance. How long before the government steps in and holds Chrysler accountable for their poor quality and repairs that fix nothing? I lost power completely. I lost the ability to maintain speed in order to safely remove myself from the road. My family could have been killed. These are family vehicles, out yourself in the shoes of someone with children, would you feel safe putting your kids in a car that could die on you at any moment without any real warning signs?

There is an active recall but the manufacturer has failed to resolve the issue in a timely fashion. They currently have no idea how to repair or replace the airbags. As a result, my family do not feel safe in the vehicle.

While driving in the carpool lane on the freeway, the vehicle displayed “Stop Safely. Vehicle Will Shut Off Soon” and lost all power. I am pregnant and had four children (ages [XXX] , [XXX] , [XXX] , and [XXX] ) in the vehicle. The outside temperature was over 100°F, and we were two hours from home. The car was completely disabled in heavy traffic, creating an extreme safety risk for my family. After waiting about 15–20 minutes, the vehicle restarted and we were able to continue. A code scan showed P0A78 – Drive Motor “A” Inverter Performance. This is a serious safety concern, and I am requesting immediate inspection and repair under warranty. On 8/12/2025, he vehicle displayed a warning that said "Stop Safely, Vehicle will shut off soon" shortly after it was parked. It also displayed a red turtle and the check engine light. There was no warning or check engine light prior to the message that the vehicle was going to shut off. Fortunately, we had already parked safely, but were on the freeway traveling and about an hour away from home. It was 100 degrees outside, and we were stranded with no help from Chrysler customer care other than to tow the vehicle, leaving our kids and dog in a parking lot with no car. We declined this option and had it checked by a diagnostic computer at Jiffy Lube, which returned no diagnostic code. Eventually, all of the warnings and check engine lights went off, and we were able to take it home. We took it to the dealership at the earliest opportunity on 08/14, and they confirmed that this is a known issue and would require a transmission replacement. There is no timeline on the repair or replacement, only that it will take "a while". They do not advise that we take the van back because the issue could come nback at any moment and the vehicle could shut off without warning.

The vehicle has been at the dealership multiple times for transmission issues, and the most recent time slammed into park while driving slowly on a side road. It is a known hybrid transmission issue and the dealership just let me know that the manufacturer will not replace the transmission, only rebuild per TSB 21-025-25. I am concerned based on research of others with this issue that the rebuild will not fix the issue and the transmission will shift to park on the highway with my wife driving.

While traveling on an interstate highway at highway speed (65-70 mph), a red turtle warning light came on with the message "Pull over Safely; vehicle will shut off soon." Almost immediately, we lost all power and glided into the right-hand shoulder. We narrowly missed being rear-ended. We got it to a local dealer, where the service department informed us we need a new transmission. The car has less than 13,000 miles on it.

My vehicle suddenly experienced a loss of power while driving, starting at 62000 miles per hour. Sometimes the same problem occurs again after driving for several hundred miles per hour, and sometimes even after driving for tens of miles per hour. When this problem occurs, the vehicle cannot continue to drive and can only rely on inertia to slide. It must be parked on the side of the road, the engine must be turned off for one minute, and the vehicle can start again to drive normally. My car has now driven at 89000 miles per hour, and this problem has occurred countless times. At first, it occurred about once every 2-3 days, but now it occurs twice a day.

[XXX] First “Red Turtle” Breakdown on Freeway After about an hour of freeway driving, car went into “red turtle mode” (“limp mode”) on the freeway, alerting us that the engine would shut down shortly and that we needed to get to a safe place to stop. The car wouldn’t accelerate fully, putting us in danger on the freeway, where cars were speeding all around us. June 2025: Dealership’s First Attempt to Fix The dealership showed stored error codes P0A43 Drive Mode Code A Position Sensor Intermittent. But, the tech said they were unable to replicate the error. They kept it in the shop for a couple days, then sent the car home with us with no fix. They told us to restart the car a few times and it should go away. [XXX]: Second Breakdown on Freeway On [XXX], with all three kids in the car, we drove the car from Sacramento to San Rafael. On the way there–the first long car trip with the van since the [XXX] incident, the Red Turtle Alert appeared again. We were going uphill and unable to fully accelerate on the freeway, again putting our entire family in danger. We were luckily able to pull off of the freeway, restart the car several times, and continue driving–wondering the entire time whether the vehicle would again lose power, and sure enough it did. [XXX]: Third breakdown on freeway Later that same day, on our way home, the Red Turtle alert appeared again, the vehicle was unable to fully accelerate, and the message warned us that the car was about to shut itself down. This time, we were on the freeway with no exit available to us. We had to pull over onto an unprotected shoulder, with cars speeding by us on both sides. It would have been unsafe for any of us–but especially my toddlers–to exit the vehicle. We sat on the side of the freeway, restarting the car repeatedly, waiting to see if the Alert would clear and the vehicle would become drivable again. Eventually, it did. We brought the car to the dealership again. Spent over a week there INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA)

This is a PHEV and upon returning to my vehicle after being plugged into a Rivian public EV charge station I found that the charger handle overheated and smoking. When I left the vehicle 45 minutes earlier it was charging with no issues whatsoever. When I removed that handle, a large piece of the charge terminal came out of the handle a remained attached to my charge port. It was one of the connecting sleeves that melted out of the charger and somehow arc welded itself to the male end of my charge port. I still cannot remove it and so now I am unable to charge my vehicle. I notified the property officials and also notified Rivian, but neither is claiming any responsibility to the damage to my charge port. I took my car my local Chrysler dealer and explained what happened but they denied any fault as well. My vehicle has less than 13k miles and still covered by new car warranty so I thought they might know why that happened. In my judgment what had happened is the charge cable itself had a weak wire or a bad, loose connection to the copper terminal inside the handle that caused high resistance and heat to build, then broke away and bound itself to my charge port once I removed the charge handle. When I try to followup with Rivian, best I can do is communicate with their front line phone customer service (they never allow me to speak with anyone authorized to make decisions about the matter), however Rivian did fix their public charger after the incident, but they're seem uninterested in assisting a fix to my vehicle, which I feel they are at fault So have claimed Chrysler had had charge port issues but they are denying assistance. I have an estimate of around $980.00 to repair my charge port, but neither Rivian nor Chrysler warranty are willing to make repair. I feel victim here, used equipment as advertised but being denied product liability claims by both parties, Rivian and Chrysler.

Took the car in on 5/10 which was the earliest appt. Said they did a software update, gave the car back. Took the car again on [XXX] when the same warning appeared again on the interstate, same situation disabled vehicle at high speeds on the interstate and no power. Said they changed a module related to the error and gave me the car back. Yesterday as I was driving on [XXX] 70 mph the car gave the same warning and shut off again, immediately I had no power, 30 mph. The dealer acknowledged my car is not the only one in his shop with this issue. They had my vehicle for nearly a month the time before. This vehicle is not safe to drive on the highways as it can shut off at any time. It's a known problem by Chrysler and their idea of fixing it, is issuing a warning which gives you almost no time to get off the highway before your speed is greatly reduced and even then you're stuck on the side of the highway with cars speeding by in an area with no shoulder or construction zone you'd be stuck in the middle of the road. Someone will wind up dead on the highway, I am lucky that in the 3 times this has happened I have not been hit and have been able to safely get to the shoulder, but I don't see Chrysler being able to fix this issue, the dealership did reach out to the manufacturer each time. I know my car is not the only one that has this issue and I'm concerned for the safety of anyone who owns one of these. A simple google search of this vehicle will show the problem is wide spread. On June 3, 2025, my 2023 Chrysler Pacific Hybrid gave me a warning message that it was about to shut down. This included a "red turtle" icon that I later learned is called "limp home mode." I was traveling on the interstate at highway speed at the time. I was able to pull over to the shoulder and turn the vehicle off. The risk of a car shutting down on the highway is a huge a safety risk to myself and other vehicles. Stopping on the shoulder of a highway, particularly given that it was early morning and not yet fully light out also felt very risky with other cars passing by. After turning the car back on, the limp home mode turned off, but the engine warning light was still on. I was able to re-enter traffic, which was also a safety issue having to deal with. When the dealership opened, I contacted them. They were familiar with the problem and were able to state over the phone that it was likely a transmission issue. After inspecting it that afternoon, they determined it would need a transmission rebuild. As of June 27, 2025, they still have the vehicle and have not yet made the repairs.

The van did not transition from electric to gas engine when driving and battery for the electrical engine was depleted. The dash board displayed a red turtle and said slow on dashboard. But it would not drive forward. I was on a hill and the van backed down the hill in reverse. I did this multiple times - turn off van, restart van and try to go up hill, reverse it down hill again. At some point, there was enough electricity for the electrical engine to kick in and I was able to drive home. After the van set for a while, it restarted and I was able to drive it to my ultimate destination 147 miles away. I researched online and found there is a recall for this very thing, but it does not include my car. Why is my life so hard? Its a new van to me.

Driving at highway speed (70 mph) the vehicle repeatedly has flashed a warning : Stop Safely Vehicle Will Shut Off Soon The engine cuts power and the vehicle must be pulled over and turned off and cannot keep driving. While exiting an interstate, the van felt like it had popped out of gear and lost power, and the instrument cluster had the message "Stop safely, vehicle will shut off soon." I was in the middle of three lanes on the ramp, and due to traffic and no shoulder I was unable to stop off the side of the road. The van died at the top of the ramp at the traffic light, causing a risk of being rear-ended by approaching traffic. I had my two children in the van at the time. After attempting to cycle the ignition several times and stepping out to wave traffic around as the traffic light cycled several times (causing a traffic backup), the van started and drove normally albeit with the Check Engine Light on. I have not had the time to take the van to a dealership for diagnoses, but looking online this is not an unknown problem and is related to the Pacifica Hybrid's transmission. Just wanting to make NHTSA aware of another event of this fault occurring.

After driving 15miles home from work, stopped at a traffic light to turn left. When attempting to proceed forward van stopped with an error code that indicated "Stop Safely Vehicle Will Shut Off Soon". The van shut down, and could not move even though it was blocking the intersection. Law enforcement had to be called as it was a safety hazard, and a tow truck had to be dispatched to remove the van from the intersection and tow to dealership for warranty service. The dealer provided warranty service as per Chrysler direction, which was a software update and reseating electrical connections. However, as per the notes from the dealer, "possible a loose connection was issue but still could be internal to power inverter module." As the repair was not a definite resolution for the issue, I contact Chrysler corporate to request a more definite repair. Their stance was that they considered the car repaired under warranty, and that they would not authorize any further work. Chrysler stated that they will not do anymore work until the issue happens again. The issue is that the condition that results is that the van shuts down - and it can happen anywhere, the middle of an intersection or on the freeway at highway speeds. Basically Chrysler has told me that even though they are not sure that the fix applied may not have remedied the issue that caused the van to shut off, they will not do what is required to ensure that it does not happen again. Chrysler has put the safety of my family second to their bottom line - by refusing to authorize the expensive work that it would take to remedy the problem, and ensure that my faily does not face a dangerous situation when driving our van.

I rented this car from Budget on Saturday, May 31, 2025. When I drove it on Sunday, June 1, 2025. The check engine light came on and the car gave a warning that it would shut off. The first time I saw this notification I was stopped at a stopped sign at a 4 way intersection. The car proceeded to shut off after some time (maybe 20 seconds or so, not sure). I was at a full stop and the car was in "Drive". When the car shut off it went into "Park" automatically. I turned the car back on and put it back in "Drive". The car would not move. I check to make sure the parking brake was not turned on and it was not. I put the car in "Park" and back into "Drive" and the car still did not move. I turned the car off a few more times before it switched back into gear normally. I was unable to return the car at that time. About an hour later as I was driving on the interstate in the left lane, the same warning light came on. I had to immediately move from the left lane of a 6 lane interstate all the way to the right shoulder. There was no exit or left shoulder. Once I got there the car idled for a moment and shut off; I am uncertain how long it was before it shut off. I waited a couple of minutes since I was also shaken up at having to move so many lanes on a busy interstate with so many lanes. I got to my destination after about 30 minutes. On the way home about an hour in to the drive, the same thing occurred. I moved onto to the right shoulder of a two lane interstate at this time. This time I shut the car off myself and waited a couple of minutes to turn the car back on. I then drove another 45 minutes home. Then this morning, same warning in my driveway before I left to return it. There were issues with the blind spot warning not working some of the time as well as the display screen would flicker on and off or just become disabled but still visible. I have reported this to the rental company but I want that car off the road.

Second row passenger seat headrest does not lock into place. After folding the seat down to put in car seats in the third row, the headrest will not lock. This would cause severe trauma if one of our many children were to be seated in the seat during a crash. The issue has not yet been evaluated at a service center, but I intend to contact them and see if the headrest can be replaced through warranty.

I received a warning to park safely, engine will shut down soon. This is the third time it has happened. One time coming down a mountain with no place to pull over safely with a van full of family members. Second time at high speed on an interstate, this time on a high speed local road, but had enough time to pull over before losing power. The van has been in the shop and/or waiting for repairs for over 10 weeks during my 53 weeks of ownership. Chrysler has delayed accepting the recommended repairs that their dealer states is necessary. I have spoken to other Pacifica owners that has experienced the same issue. Is Chrysler waiting for folks to get killed before accepting responsibility?

Letter sent to AVIS after the incident: The image included is an example and not associated with incident Concern Regarding Recalled Rental Vehicle - Avis Rental Agreement [XXX] Dear Mr. Tucker, I am writing to share a concerning experience my wife and I had with an Avis rental. We rented a Chevrolet Pacifica Hybrid for a drive from Atlanta to Chicago. About an hour into our trip, the vehicle displayed a warning message: "Stop Safely. Vehicle will shut down soon." Following the instructions, we immediately pulled over, activated our hazard lights, and called Avis emergency roadside service while consulting the owner's manual. We then decided to move to a nearby gas station parking lot for safety. While still on hold, we researched the warning online (see link) and discovered it was a known recall for our vehicle. The recommended temporary fix—turning off the car, opening and closing the driver’s door, and restarting the engine—allowed us to continue. However, the warning appeared three more times during our trip, requiring us to stop and repeat the process. Though we managed the situation, it caused unnecessary stress and could pose a serious risk for other customers. After 15 minutes on hold, we disconnected from emergency road service as we no longer needed assistance. We called back later to inquire about switching vehicles, but chose to continue rather than delay our trip even further. This raises important questions: Why does Avis place vehicles with outstanding recalls into its rental fleet? What is Avis’s general approach to managing manufacturer recalls? If such vehicles are rented, why not inform customers in advance and provide a workaround? While I have contacted Avis/Budget about customer service concerns in the past, this is a safety issue. I urge you to escalate this matter to your management team and clarify how Avis plans to prevent similar situations. I would appreciate a formal response outlining your approach to recall management. We were driving on the freeway, 80 MPH, 0% battery life. Warning came on the dashboard stating something like “ Pull over safely, engine will stop”. Engine had already stopped and we luckily avoided impending doom and pulled off to the side of the highway. Turned car off, waited a couple min, and started the car again. Car turned on with check engine light. Able to slowly get to the next exit. Next morning, took to dealer. Say there were not able to see any issues. Van is around 28,000 miles.

I have completed recall 66B/NHTSA 24V-436 on my Chrysler Pacifica. I'm continuing to have issues where the uconnect system locks up and restarts, the rear view camera freezes or turns on when driving forward. Once the system locks up you can't operate certain vehicle controls such as HVAC as they are controlled through the system and the buttons do not respond. I tried contacting Uconnect but their phone system says "error" after multiple steps to get to a representative. I've read many other people having these issues online also. This is a serious safety issue if the controls for your vehicle lock up while driving and the rearview camera doesn't work. I've updated the system per the recall and the issues persist.

The vehicle randomly went into limp mode. The check engine light came on with error code P0A3F. The vehicle was restarted and drove normally with the check engine light (CEL) still on. We immediately made an appointment with the dealership to get it looked at. As we were driving into the dealership parking lot the CEL cleared itself. There is an existing TSB for this exact scenario and symptoms. The dealership and Chrysler corporate (Stellantis) are refusing to fix this issue because there isn’t an “active code.” The code is in the history. Since then the CEL has come on multiple times but cleared itself too quickly to get it to the dealer in time. We took it a second dealership this time with an active code, but the code cleared itself upon restarting the car after leaving it there. This vehicle is not safe to drive since it randomly goes into limp mode with no power. This poses a very dangerous and unsafe situation that can endanger me, my family, and other drivers around me.

While driving at highway speed the vehicle flashed a vehicle and I’ll shut down sign. A red turtle flashed and the vehicle began to decelerate. Had to pull over to a less than safe sand shoulder. On our daily commute the car came to a stop at a light and then the car then turned off and would not start again. We tried everything but the engine would turnover. After 3 hours, the car then started and we drove it to dealership and they detected no faults or codes.

Vehicle lost power on three occasions while on the highway. STOP SAFELY VEHICLE WILL SHUT OFF SOON showed in cluster and began to lose all power. I had to pull over and turn off vehicle for about 10 minutes to get it to move again. Vehicle was taken to dealer and they reported the transmission needed replacement and was covered under warranty.
