My 2021 Chevrolet Tahoe suddenly shaked and the engine shut off and on and nearly stalled on the road. I towed the car to the servicing dealer who has not been able to fix the issue. There is a national recall for the issue for the month of September 2021 Safety recall N212332040 for this issue but NHTSA has not added my car. NHTSA must include my car with the same issue and built in during the same month of the recall. The safety issue is the car will stop running and may block traffic or can be hit when it breaks down.

The contact owns a 2021 Chevrolet Tahoe. The contact stated that while his wife was driving at undisclosed speeds, there was an abnormal ticking sound coming from the vehicle. The check engine warning light was illuminated. The vehicle was taken to the local dealer, where it was diagnosed, and determined that the cylinder #1 left side lifters and push rod had collapsed. The lifters and push rod assembly were replaced. The contact stated that on a separate occasion, while driving at undisclosed speeds, there was an abnormal ticking sound coming from the vehicle, and the check engine warning light illuminated. The vehicle was taken to the local dealer, where it was diagnosed, and determined that cylinder #4 lifters had detached, and the push rod was bent. The vehicle was repaired. The contact stated that the engine was consuming an excessive amount of engine oil. The contact related the failure to TSB: 15-06-01-002A. The dealer and manufacturer were not made aware of the failure. The failure mileage was approximately 12,400.

I have a 2021 Tahoe that while circling a parking lot at my son's football practice. The Tahoe's emergency came on and the Tahoe abruptly stopped. A checked engine light came on. The Tahoe wouldn't release the emergency brakes. I was finally able to get it released. I took the vehicle to a transmission shop because I plugged OBD scan tool on it. It had two transmission codes. One generic and one for the vavle body. The transmission shop said they have had lots of Tahoes with the vavle body's going out. They told me about N242454441. I called Chevrolet and they advised me that wasn't for my Tahoe. I asked why and they said they don't know. If this is effecting the desiel only models then why is mine doing the same thing. Parts cant be ordered either. Please look into this matter.

This is an amendment to campaign 11684444 as there is some inaccurate info as it pertains to the diagnostic- 2021 Chevrolet Tahoe, Driving 25 MPH, in the neighborhood, my Tahoe kicked into neutral and abruptly stopped in the middle of the street. I immediately tried to restart it, however it would not start. After about 15 minutes, we tried again and it started enough to make it back to my in laws house, where it remains parked today. We did a diagnostic test and received the code for Fuel Pump Control Module failure. Contacted the Chevrolet dealership I bought my vehicle from and they indicated this is a common issue and the part is on back order. This is a very dangerous risk to safety and should be covered under a recall. I've seen where GMC issued a recall for the exact same issue, but only for Tahoes with 6.2L V8 engine; Mine is 5.3L. I am confused why that matters as it poses the same safety risk across the board. Very dangerous. Please consider this as a recall.

When I purchased the car the wrong oil was in the car which always burned off with 2 months after an oil change. They figured out the right oil but the damage has already been done.

After analysis, they stated the transmission valve body and electronic control component needs replacement over $3,000. after tax. No warning signs prior. NTSB recall N24254441 covers only diesel engines not gas with same problem in 10 speed transmission.

On August 23, 2025 while driving on a highway with my daughter in the backseat, my car gradually stalled while it was going through an intersection. It ended up stalling prior to a turn ramp onto another highway. This resulted in an unsafe condition and unsafe location as it happened without warning or any alerts prior to or during the drive. The Tahoe was towed to the dealership service repair shop (Hendrick Chevrolet in Cary, NC) and was found to have a fuel pump control module failure requiring replacement (replacement: FP-number 13554821, module 3.900). The dealership service department acknowledged this is a part failure on the manufacturer end; however, mentioned this is a safety bulletin, but not a recall for the part. Due to the Tahoe being outside of warranty timeframe, this ended being an out of pocket expense. The service staff told us there were four other Tahoe vehicles waiting for this part at the dealership, and we waited over a week to obtain the part as it had to come from the manufacturing line. Due to the timeframe for repair (>1 week), we had associated rental car payments that was not provided by or covered by the dealership/manufacturer.

I am writing to formally document serious engine defects I have experienced with my 2021 Chevrolet Tahoe equipped with the 5.3L L84 V8 engine. 1.Crankshaft Failure oMy Tahoe suffered a catastrophic crankshaft failure that required major repair/replacement. oI am aware that GM has recalled certain vehicles equipped with the 6.2L L87 engine for crankshaft and rod-bearing defects. oBased on my experience, the same or a very similar defect has also occurred in my 5.3L engine, which is currently not included in that recall. 2.Valve Lifter Failure oIn addition, my vehicle has suffered from valve lifter collapse—a well-documented problem in 2021–2022 GM trucks and SUVs with both the 5.3L and 6.2L engines. oThis caused severe misfiring, drivability issues, and expensive repair needs. Why this is a concern: •Both failures occurred well before what would be expected of a modern engine. •The crankshaft failure raises a safety risk: sudden loss of propulsion while driving could lead to accidents. •The lifter issue is widely reported by other owners and has been the subject of GM service bulletins. •Taken together, these problems suggest systemic defects in the 2021 Tahoe’s 5.3L engine that have not been adequately addressed by GM. The first occurrence, my vehicle was running and parked in the rain after driving approximately 5 miles. While parked my check engine light came on with a series of warnings: service ECS, service traction control, forward collision system unavailable, reduced acceleration drive with care. Two days later it went away. The second occurrence, I was driving in the rain for approximately 22 miles and briefly parked with my vehicle running. I then turned out of a parking lot and began to accelerate to speed up and the check engine light came on along with all the same messages, this time my vehicle acceleration had seized and wouldn’t go faster than 30 MPH. I slowly drove back to the parking lot and turned the vehicle off to let it sit, disconnected the battery and reconnected, and the issues went away. The third occurrence, I was driving on the highway in the rain for approximately 10 miles, I went to merge into a right hand lane, I took my foot off the acceleration peddle to slow down for the merge and as I slightly turned the wheel to the right, my vehicle spun out three times, throwing the back end of the Tahoe into the inner highway wall, which spun me again to face the right direction, next to a highway exit. I then went to accelerate to exit the highway but noticed my vehicle would only coast and realized my engine had cut out at some point. I started the vehicle and exited the highway and parked in a parking lot only feet away from the exit. After I parked I noticed all the same warning lights came back on. Chevrolet/GMC knows that these modules will make the vehicle stop wherever it is driving. I happen to be on a freeway going 70 mph and could have been killed. Dealer replaced with GM Part #13554821 On each failure, vehicle was unable to shift to neutral, power remained on, electrical component began shutting down to conserve power, left stranded and required towing with rear wheels being dragged locked up onto tow truck. Once power was shut down, could not operate windows or lock doors other than keyed driver's door. Th entire failure mechanism puts the driver in an extremely unsafe situation. Recall notice #21V739000 was issued on 9/23/21 with to impacted notified owners on 10/14/21. Did not receive this notification since first failure was prior to recall and assume GM considered vehicle repair completed. Note that the fuel module part number has changed 3 times in my case, but Part # 3554821 replaces 5 other listed part numbers. Is this the 6th iteration or of this part? What is the root cause of the fuel module failure? Is it the module itself failing or an electrical issue that causes the fuel module to short out and fail. GM/Chevrolet is not solving this issue. With 3 failures of the same component, this is a lemon and I have no comfort or confidence using this vehicle. Dealer replaced with GM Part #13554821 On each failure, vehicle was unable to shift to neutral, power remained on, electrical component began shutting down to conserve power, left stranded and required towing with rear wheels being dragged locked up onto tow truck. Once power was shut down, could not operate windows or lock doors other than keyed driver's door. Th entire failure mechanism puts the driver in an extremely unsafe situation. Recall notice #21V739000 was issued on 9/23/21 with to impacted notified owners on 10/14/21. Did not receive this notification since first failure was prior to recall and assume GM considered vehicle repair completed. Note that the fuel module part number has changed 3 times in my case, but Part # 3554821 replaces 5 other listed part numbers. Is this the 6th iteration or of this part? I was told by a Chevy Service Technician that it can take 2 weeks or 2 years for GM to find a “remedy”. my fuel pump failed while driving on the highway with my whole family. i lost all power and my gas pedal lost all compression. I was barely able to pull off the road safely. no indicators flashed on my dash, no warning just a total loss of power and control. after pulling over, i let the truck sit for a second then tried to restart. it wouldn't. at that point, the yellow check engine indicator turned on. I logged into MyChevrolet and Onstar to see if any notifications were available to help diagnose, but nothing stood out. all systems appeared to be perfect at last check. I check temp and it was normal, oil level, normal. battery level, normal. Key battery, normal. i then started to google why my 2021 tahoe would die while driving and came across and widely known recall for 2021 GMC Full Size SUVs including Tahoes with faulty fuel pumps. my issue matched exactly to the description of what would occur. I did a quick search to see if i missed a recall notice for my truck, and didn't see anything. I was able to get my truck home and then to a dealership for a diagnostic. the only code that was discovered was for the fuel pump/fuel pump module. The dealership is now stating my vehicle doesn't qualify for a recall repair because my VIN doesn't appear on the affected vehicles list and that repairs will total $859.68 out of pocket. however, they also stated that early builds for this year may not have enough evidence. i ordered my truck, built to spec 6 months prior to receiving it. i would like my VIN added to the recall list. I have kept immaculate care of this truck and this is a known safety issue. this is my family moving rig. I don't want it to be unsafe.

On April 25th, 2025 while traveling on Interstate 20 at 70+mph speeds with my children in my vehicle and 30 weeks pregnant, with no indication of an issue my 2021 Chevrolet Tahoe suffered catastrophic engine failure. There was zero warning from the vehicle including noises/electronic display warnings that this was fixing to occur. In the middle of going 70+mph surrounded by traffic on a busy interstate the vehicle just shut off completely. I was able to use what momentum I had from my speed to get on to the shoulder. However, the failure prevented me from being able to turn on my signal to alert other drivers of my merging and prevented the use of my emergency flashers to signal once on the shoulder immobile. The vehicle would not start up again after that, and I had to call a tow truck to take my vehicle to the nearest certified Chevrolet service center. When the tow truck arrived the vehicle would not even shift into neutral allowing them to load it without potentially causing more damage. It had to be drug with the wench in park onto the tow truck bed. Once at the nearest service center I was told it would be up to 3 weeks + before they could do a diagnostics. I was 14 hours from home when this happened, and now had to pay to have my Tahoe towed 14 hours back to the service center near my home. On May 8th the failure was determined to be caused by this recall, specifically the crankshaft. The dealership has lied and said they've ordered a new motor and it came in, then called to tell me they ordered the wrong motor. They said they ordered the correct one and it would be in within 14 days, and that was 25 days ago. They called today June 16, 2025 to tell me there is still no update on when my engine will ship to complete this repair. The dealership has also refused to assist me with any loaner vehicle or rental assistance even though GM has told me I am entitled to it under this recall. GM has moved my case to Parts Delay team and I still have not been contacted.

We were driving our 2021 Chevrolet Tahoe (purchased Dec 2020; odometer currently right under 40,000 miles) out of town this past weekend and I noticed the headlights were not illuminating the road ahead very well once the sun went down. They seemed like they were pointed down and when i turned on the high beams, those were also lower than how the low beams are usually pointed. We were stuck in the middle of nowhere and I persevered with low lighting onto our destination. After researching the issue on the internet days later, I found a Tahoe Owners' forum/thread [XXX] ) where several other 2021 Tahoe owners had experienced the exact same issue as ours had. The culprit appears to be a sub-component of the headlight adjuster mechanism, but GM/Chevrolet does not offer a replacement for either this small white plastic piece that is failing and the only repair option they have for this issue is complete replacement of the entire headlamp assembly ($1,350 MSRP). This is a dangerous problem that occurred randomly on a vehicle with low mileage and appears to potentially be a common failure for other owners of the same headlamp design. I have not taken this vehicle into the dealership for inspection, as I expect they will only replace the entire headlamp assembly for $$$$. The contact was then informed that the engine lifters needed to be repaired. The manufacturer was notified of the failure and parts were ordered to repair the vehicle. Although these components were replaced, we were informed on May 15, 2025, that the vehicle continues to misfire and now requires a camshaft replacement as well. The total cost of the repair has now risen from $5,182.16 to $13,549.79, which is a significant financial burden as the vehicle is just outside of warranty coverage. We are aware that there is an active recall covering similar issues for 2021–2024 Chevrolet Tahoes equipped with 6.2L engines. However, our 5.3L engine model is experiencing the same mechanical failures, including misfires, knocking noises, low RPM idling, and performance degradation. There is growing evidence that the valve lifter issues are not limited to the 6.2L engine variant. Our experience—and the experiences of others reported online—suggests that the 5.3L L84 engine may also suffer from the same design or manufacturing flaws. We respectfully urge NHTSA to investigate whether this issue warrants expanding the scope of the existing recall to include the 5.3L engine models, and we ask that GMC/General Motors be encouraged to provide assistance. Given the severity and known pattern of these issues, we believe GMC should cover at least 75% of the repair costs, or ideally, offer to perform the repair at no charge. Check engine light comes on. Car suddenly stalls and engine shutters, shuts off and on. Goes into limp mode as engine shutters at about 15 miles per hour. GM has not included this 2021 Tahoe which is the same exact condition under Product Safety Recall N212332040 "Stall While Driving". That recall was issued at the same time or month as this Tahoe was built and sold and should have been included. This Tahoe was towed to the dealer and GM has not been able to supply parts. The Tahoe will soon have been at the dealer for 60 days with no solution. The recall build dates should be expanded to include all GM vehicles with this defect. The vehicle was towed to Redline Performance LLC, where it was diagnosed that there were metal shavings in the oil pump, camshaft, and in other unknown parts, along with the lifters failure. The mechanic replaced the lifters, the connecting rod, the camshaft, the oil pump, and other unknown parts; however, the failure recurred after picking up and driving the vehicle to the residence. Additionally, there was smoke coming from the engine. The vehicle was towed to Stan McNabb Chevrolet Columbia (101 S James Campbell Blvd, Columbia, TN 38401), who informed the contact that the vehicle would not be diagnosed by the dealer. The contact became aware that the VIN was included in NHTSA Campaign Number: 25V274000 (Engine and Engine Cooling), which the contact related to the failure; however, the part to do the recall repair was not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was notified of the failure, but no assistance was provided. Luckily I did not have my children with me. I called 911 and a police officer came and parked behind my vehicle with his lights on so hopefully no other vehicles would hit it. A tow truck was immediately dispatched as high priority by the police. I had it towed to Hedricks Chevrolet dealership in Clovis, California, who informed me there was metal parts in the oil and that the engine was destroyed and had to be replaced. It took several months to get the engine replaced and in the meantime, I bought a new 2023 Cadillac Escalade so I would have a vehicle to drive. The Cadillac Escalade ended up having an extremely similar problem (and I will file a separate report for that vehicle). A few months later when the Tahoe replacement engine was installed I picked up the Tahoe from the dealership and sold it to Carmax.

My vehicle has a little over 15000 miles on it. On Thursday April 3rd, while driving 35 mph in town, my RPMs shot up to 4 and the vehicle would not respond to accelleration. I took to dealership on 9/13/22 (5months after getting vehicle) and the codes P03000 00 which is an engine misfire, P050D 00 which is cold start rough idle and it was noted by service tech that engine making a clunk and ticking noise. There was a misfire on cylinder 6. They removed the valve cover and the intake valve rocker arm was not moving at all and when the rocker arm was removed they noticed the push rod was also bent. They verified the DFM lifter was collapsed.

I was driving the vehicle, exiting the parking lot when the engine and all electrical components turned off. This could have easily caused and accident as there were multiple vehicles that had to go around me while I tried to restart the vehicle. When I finally got the vehicle towed several days later (which was not easy because in order to put the transmission in neutral without the engine being on, a special tool is needed) I took it to the dealership. They confirmed it was the fuel controller that went bad for the second time. The first time the vehicle was stuck in my garage at home with the same symptoms. They apparently replaced it with another faulty part that is a ticking time bomb. I have no faith that this won't happen again.

After I washed my 2021 Chevrolet Tahoe, I went to run some errands. As I pushed the start button, the car would try to start but did not. I tried several times without success. I had the truck towed to Ed Morse Chevrolet in Sunrise, Florida. My odometer is at approximately 43,000 miles. About 3-4 months ago, at 42,000 miles, I had the same situation. I had to have the truck towed to Ed Morse. At the time, the Service Writer told me it was probably the fuel cell module and that Chevy has been a problem with them. The Service Writer surprisingly admitted that this issue could cause the truck to stop in the middle of your commute! I told him that an issue this serious needs a recall. So, today, when AAA picked up my truck, he said that this happens frequently and he towed many Chevrolet Tahoes and pickup trucks with this issue. The AAA technician even reiterated that the fuel cell modular failure shuts down your vehicle while driving! This is insane and troubling. Within 1000 miles, I will need 2 fuel cell modules and there is no guarantee that this one will last. Battery charge/jump would not work...watched youtube videos and someone said to try disconnect/reconnect cable and it worked to start it up at that time & did take it into Dealership after that incident and no-cost repairs were made for that issue. Approximately 4/14/24 while I was driving home from being out of town with my child the check engine light came on with a message “Reduced acceleration drive with care.” The car was not shifting and I couldn’t go over 45 mph and the rpm’s were about 3000 at that speed. The light went off and the car seemed to be driving normally. While on the interstate traveling at approximately 70 mph the car did a hard shift and drastically slowed while the rpm’s spiked. I took it to the dealership who determined the valve body needed to be replaced. Approximately August 2024 it happened again but the light went off. December 2024 same issues with the check engine light, reduced acceleration and not shifting happened. Took the car back 12/9/24 where a wire was replaced. Picked the car up 12/12/24 and 12/18/24 while driving in the car rider line picking my child up the car had a hard shift, check engine light came on, reduced acceleration message and the car would not shift. Notified the dealership immediately and took it back 12/26/24 where they did a second valve body replacement. 1/28/25 on my way home from running errands and was stopped at a red light and the car shifted hard, check engine light came on, reduced acceleration message and engine would not shift causing me to drive slowly to get home (40 mph or less). Dealership was notified immediately and took in the following day. Dealership had a week and had me pick it up because they “couldn’t replicate the issue.” 2/15/25 after driving 2 hours we pulled in a parking lot and the car shifted hard hard, check engine light came on, reduced acceleration and would not go into reverse. The dealership picked the car up and have had it since trying to replicate it. It occurs without warning while driving and will drastically slows the car down putting us a risk of being rear ended due to the rapid and speed change. It also is a safety risk due to lack of warning that a power failure is about to occur while driving.

Upon taking to dealer, service advisor mentioned that it could be valve body issue. which is very common for these vehicles. After diagnosis from dealer, there were two codes: P0770 and P0776. Upon some research it was found that these issues are indeed very common for 10speed GM transmission. But, GM hasn't done much to address it. GM acknowledged the issue in their Oct, 2023 bulletin. This is a safety issue especially at highway speeds.

Since the vehicle determined I didn’t have air in one of the tires (spare previously mentioned) the four wheel drive shouldn’t work to prevent damage. I needed the four wheel drive in the snow so I had to proceed at approximately 25 mph. This is a safety issue for not only my vehicle but others on the roadways. I contacted the dealer that had no idea about the problem and neither did the national GM customer support center. There is a tool that “tells” the vehicle where each tire is located so that is low tire pressure occurs the driver would know which tire needs attention. If a TPMS was installed from the factory and a flat occurred, spare installed then the four wheel drive would function. If a flat subsequently occurred the driver would still be alerted but wouldn’t know tire was involved. Visual check would find it. Either this or have GM provide an override so the trip could be continued safely. I believe that this situation applies to any four wheel drive vehicle not just GM and should be resolved if the NHTSB would initiate an investigation.

2021 Chevrolet Tahoe Engine began misfiring and check engine light and ESC warning indicators displayed as I was driving. I pulled over and shut off vehicle, read manual and restarted engine. Same problems occurred, so I drove to Chevrolet dealer which was a mile away. The advisors said I shouldn’t drive it on the expressway or any further since engine had known problems that GM was aware of and it would blow up. Scans showed P0300 & P0305 for Cylinder 5 misfire. Major Repair included replacement of all lifters and push rods on both banks and the camshaft taking 17 days. This engine should have a GM Recall due to its severity. I was told the engine usually blows up before the warranty period is over and GM would just fix it. Unfortunately for me, my engine failed after full warranty and while I was traveling out of state to a family wedding, costly me directly. GM did pay half of the cost of repair which indicates admitting fault. The major safety concern is when an engine blows up and the driver cannot control the vehicle causing accidents or fire (and being stranded). There were No previous indications or symptoms of engine failure prior to date the problem occurred. Police departments in Illinois have had the same problem with this engine and their entire Tahoe fleet needed a new / repaired engine. This is an unacceptable action by GM to just wait for the problem to occur instead of proactively recalling the engine for repair.

This is a fairly new vehicle, so to be told we need a new engine when we’ve had the car for only 4 years is ridiculous! We were driving one day and heard a knocking sound coming from the front of the car. We made it home, but when we tried to leave the next time, we only made it halfway down our driveway when we heard a loud knocking noise from the front and the car suddenly stopped. We were dead in the water and needed to get the car towed back to our house. The gear shift was also stuck in drive and we couldn’t turn the car on. The engine had completely seized up. The tow truck driver had to get underneath the vehicle in order to disengage the gear shift and allow us to shift into neutral so that we could get it back home. When we had it towed to our mechanic, they told us that it was a bearing failure and the connecting rod had caused our engine to seize up. I’m just glad we weren’t on a major highway or busy street at the time this took place. This is supposed to be our family vehicle and it has been out of commission for more than 6 months because we haven’t been able to afford to replace the engine. We’ve tried contacting GM directly, but they said we were beyond our warranty and there was nothing they could do to help us.

Unable to Shift Soon." multiple times, specifically on [XXX] and again on [XXX]. This issue is particularly concerning as there is an existing recall on the 2021 Tahoe for a transmission problem that can cause the rear wheels to lock up or result in the vehicle suddenly losing the ability to shift, as explained by a GMC transmission technician. Attempts to Resolve: I have contacted multiple dealerships to attempt to get my transmission diagnosed. Unfortunately, all dealerships within my area are backed up with transmission work for 4-6 weeks. I have spoken to multiple dealership service departments and have been informed that there are numerous vehicles currently experiencing this same exact issue. Some vehicles are continuing to drive, while others are being stranded on the side of the road, unable to shift. Safety Concern: The safety concern, as stated in the recall, is that my rear wheels may lock up or my vehicle may suddenly lose the ability to shift. This is particularly alarming for me as I have to drive over a large steep bridge every day to go to work, and there is no emergency lane while traversing the bridge. I am terrified to continue driving, but I only have one vehicle for my family (this 2021 Tahoe) and no dealership has a loaner vehicle available due to the high quantity of transmission errors.
