General Motors LLC (GM) is recalling certain 2025 Chevrolet Equinox EV vehicles. The pedestrian alert sound system may have incorrect software and not produce sound at a sufficient volume. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 141, "Minimum Sound Requirements for Hybrid and Electric Vehicles."
Risk: A vehicle that fails to make sufficient sound may result in pedestrians being unaware of an approaching vehicle, increasing the risk of injury.

Glare on the windshield from the reflected dash can be quite profound on sunny days. At times, when the sun is at a particular angle during late afternoon, the glare makes driving in congested city traffic very difficult due to a nearly opaque view through the glass. A perfect image of the dash blocks the driver's view of near objects, especially people walking on front of the vehicle.

The onstar module in the vehicle has not worked in two months. Its the only way the vehicle can receive updates and many safety features linked to it. No Help or response from GM or Dealership. They just say we don't have an ETA and its a waiting game. Note, no software updates can be installed without this service.

I purchased this vehicle new. Within weeks started having issues with the OnStar system. Obviously with the module not working, other things began happening. I can't see status of charge. I can't find charging stations when driving. My hands free doesn't work because google is always off line. Can't use OnStar system because it doesn't work. Came out one morning to find my vehicle unlocked and all the windows down. I live in Florida, have never once used the defrost system or set the vehicle temperature above 72. Turned the car on one day and the defrosters magically turned on and the temperature went up to 76 full blast. The the forward braking system started slamming the brakes when I was already stopping. One day it slammed us so hard that my grandchildren screamed because their car seat belts cut into their shoulders. Contacted OnStar numerous times. Contacted dealerships numerous times. After weeks and weeks, I finally have a dealer willing to look at it. I have been provided one fault code by OnStar. There are so many people with issues and research shows the module is and has been on back order for months and months. Vehicle also makes noises like it is trying to lock or unlock whenever I open a door. There were no warnings for any of it. Just began. Safety is at risk because vehicle malfunctions. OnStar confirmed there is an error. Going to dealer finally this week. No warning lamps and issues got bad enough for concern approximately one month ago but were noticed within weeks of purchasing vehicle which was purchased [XXX]. The glare from the dashboard into the windshield blocked my view almost completely when driving on the San Francisco International Airport ramp, especially when the car was on a downhill ramp. I could not see anything in front of me for a couple seconds. This could have led to an accident should the car infront of me had breaked and stopped. The glare is also visible in normal driving conditions on sunny days and can distract my attention from the road.

BRAKING FAILURE on 2025 Equinox EV LT model - my car has had two separate brake failures going down a steep dry rocky road in clear weather, once with one pedal driving on and once without, on July 21st, 2025 and August 11th, 2025 respectively. It doesn't happen every time going down this road and each time at no more than 5 - 10 miles per hour, with the brake pedal to the floor the car was not slowing down and the weight of the car on the steep track made it speed up considerably which made it dangerous due to the steepness of the hill and turns in the road. The first time I drove off the road onto flat grass and the car came to a stop, the last time it happened I pumped the brakes until the car stopped and the "ABS engaged" light came on the dashboard above the steering wheel. It happened twice on the road that time. There were no prior alerts or warning lights. The car has 5000 miles and is still under warranty, the Chevy dealership had the car for 3 days. The dealership was not able to reproduce or confirm the problem with the brakes. The dealership told me they did a series of resets. They have not given me the written service report yet. I have an appointment to take the car back to the dealer. The brake fluid level is good, there are no current alerts on the dashboard while driving. The car is available for inspection upon request. I am concerned that my wife and I may be injured or killed in a braking failure or that we may injure or kill others or damage property.

Dealer was unable to replicate issue after two attempts (ran through car wash, hose over windshield, ran A/C for extended period, checked AC condensate drainage). Dealer dried up water and carpet and told owner to monitor. Owner pulled up carpet to do own investigation after the second failed attempt by the dealer. Confirmed dry before starting investigation on warm sunny day (no rain). Owner found leak in passenger footwell underneath black access plate that only appeared with A/C usage AND during/after driving, not while car was stationary. Owner brought vehicle back to dealer. Dealer removed black access plate and confirmed leak was coming from a compromised body seam weld. Dealer repaired the failed body seam and no water intrusion has been seen by owner since repair was completed. Owner still has concerns about underlying cause of where water was coming from - theory is that condensation is forming ON the the A/C lines and is pooling somewhere in the body then sloshing around while the car is moving, causing further potential for water related issues, including rust.

The air conditioning (AC) condensate drain system is either blocked, misrouted, or improperly sealed, causing AC condensation to pool and leak into the front passenger-side footwell through the factory floor sealant. The vehicle is available for inspection upon request. How was your safety or the safety of others put at risk? The continuous leak has caused significant mold growth on the interior insulation behind the passenger footwell, creating a potential health hazard for occupants, especially those with respiratory conditions or allergies. Prolonged exposure to mold inside a sealed cabin environment poses a serious risk to occupant health. Has the problem been reproduced or confirmed by a dealer or independent service center? The vehicle was brought to a dealership for this issue, but the problem persists. I later reproduced the issue myself by running the AC for 30 seconds, after which water visibly seeped through the floor seam. Mold growth was also confirmed behind the interior trim. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The dealership previously inspected the vehicle but failed to resolve the issue. No other parties (e.g., manufacturer representatives, police, or insurance) have inspected the vehicle at this time. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? There were no warning lights or messages. The only symptoms were damp carpet in the front passenger footwell and a persistent musty odor, which appeared prior to discovering the mold and leak source.

Issue Description: While driving during daylight hours, especially when the sun is positioned at a certain angle, I have observed an intense reflection from the dashboard onto the inside of the windshield. This reflection is bright and widespread, directly obstructing my field of vision and making it difficult to see the road ahead clearly. The glare is particularly noticeable on clear or sunny days and significantly increases the risk of an accident. This visibility issue appears to be related to the dashboard material and design, which may not adequately diffuse or absorb sunlight. I believe this condition may affect other drivers as well, potentially leading to widespread safety concerns. The volume control in my vehicle will randomly change volume, up to and including full volume. This has almost caused a few crashes because it is very startling and the only way to lower the volume is to use the steering wheel controls (the volume knob does not accept input when this happens - it has a mind of its own). It is relatively simple to fight the volume when going straight, but it is very hard to do when it happens while trying to maneuver while the wheel is not centered as the controls are not accessible. While always annoying, it is very dangerous when trying to make such a maneuver and losing all concentration by blaring radio or attempting to move my hands on the wheel mid turn. This was a problem from the first day we bought the car and it was noted that my local dealer could fix it under warranty (I purchased from an out of town dealer). That dealer told me that GM was refusing to fix anything, insisting that I wait six months to see if a seemingly unrelated software update fixes the issue. The manager at my dealership has been trying to get approval and did get approval to replace a part in attempt to fix it. That did not work and they are now refusing to replace the other part that has fixed a similar issue for others. I believe they replaced the module that processes the sensor information, but are refusing to replace the sensor (volume wheel) itself. This car would have been crashed by many other drivers who could not cope with such in-vehicle distraction - this includes my daughter who refuses to drive the vehicle and really doesn't like riding in it. GM needs to fix this ASAP for myself and any other experiencing the issue. I do have videos of the behavior available, but can not attach them here because they are not accepted. It has been over 2 months since the dealer initially looked at the issue and the warranty repair was denied.

When the vehicle is in one pedal mode, and the car drive is switched to reverse, the car moves backwards and does not slow down contrary to one pedal mode which does not move unless the accelerator is depressed and will slow down when in forward drive. The 2023 Chevy Bolt EUV does operate in One Pedal Mode in both directions. This is a dangerous situation for EV drivers since the car can backup over a person since the car says it is in one pedal mode but it is not functional in reverse. EV's should not have to simulate torque convertors of ICE vehicles since that was a mechanical issue. This is completely not necessary for the car to move backwards on it own. Period! This behavior is very dangerous and should be corrected in software update to the user.

I recently purchased a 2025 Equinox EV LT1 vehicle. Overall, the vehicle is performing well and has a number of safety features that are helpful when driving. As others have also reported, the windshield glare on this vehicle is a weak spot, and without polarized sunglasses, the glare is often severe and distracting. At night, the use of sunglasses is of course not feasible, and occasional glare from headlights and streetlights can compromise the driver's ability to view the road and surroundings. Aftermarket dash covers are available, and that may be what I have to resort to to address the problem until/unless a recall or remedy is available from GM directly. This may seem more like an annoyance than a significant safety issue, but when you are unable to clearly view the environment around you when driving, it is a real problem.

Yesterday while approaching a hairpin exit, all of a sudden lights went on for the Stabilitrack, Collision alert and a number of other lights. Messages popped up saying braking assist was not available, cruise control was not available. Basically, the braking assist was gone and I had to press REALLY hard on the brake pedal to get any slowdown. All regen was gone and it was like driving a car without power brakes. I also got a message saying it would not go faster than 62mph. There was no indication of any issues prior to the Brake assist failure. This put me at extreme risk because it was essentially a power braking failure and it was extremely difficult to slow down. I had to really push hard on the brake pedal. I was able to drive my car back to the dealership where it is sitting now, waiting to be looked at in the morning of 3/24/25. I have attached a PDF of the OnStar diagnostic report, cryptic as usual, but it is a record of the codes that were thrown. Also seems like some modules were not communicating properly. I have also attached some photos of the error messages that were showing this morning.

Yesterday while approaching a hairpin exit, all of a sudden lights went on for the Stabilitrack, Collision alert and a number of other lights. Messages popped up saying braking assist was not available, cruise control was not available. Basically, the braking assist was gone and I had to press REALLY hard on the brake pedal to get any slowdown. All regen was gone and it was like driving a car without power brakes. I also got a message saying it would not go faster than 62mph. There was no indication of any issues prior to the Brake assist failure. This put me at extreme risk because it was essentially a power braking failure and it was extremely difficult to slow down. I had to really push hard on the brake pedal. I was able to drive my car back to the dealership where it is sitting now, waiting to be looked at in the morning of 3/24/25. I have attached a PDF of the OnStar diagnostic report, cryptic as usual, but it is a record of the codes that were thrown. Also seems like some modules were not communicating properly. I have also attached some photos of the error messages that were showing this morning.

dashboard reflection on the windshield: When I am driving, the sun hit the dashboard and back up on the windshield , I see the dashboard on windshield make very hard to see on the road. The top of the dashboard has a mesh-like pattern that reflects sunlight onto the windshield. When the sun is out, this pattern becomes visible on the windshield, making it almost impossible to see the road ahead. Depending on the time of day, and the direction you are traveling, the glare is very very bad. It is so bad that it obstructed my vision, the majority of the time I couldn’t even see it. Very unsafe at times - especially with any hills (even very small ones) or driving into light.

Because of the design/materials on the dash, in sunlight there is a very large amount of glare reflected onto the front windshield. Depending on the situation, it can make it almost impossible to see out of the windshield. I almost changed lanes into a car next to and about half a car length ahead of me because I could not see it because of the glare. During that same drive, I went around the corner, and faced into the sun, and the reflection from the dash onto the windshield was so bad I was blinded and could see nothing but my dash being reflected on the windshield.

Windshield wiper stops too far from the edge, a 4+ inch gap from the A-pillar (which is already wide). With snow or heavy rain showers the blind spots on the driver side are quite large. Visibility from the bottom of the A-pillar is particularly bad to begin with and made worse with the wiper gap.

Five times since I bought the new car in November 2025, the car has suddenly slammed on the brakes. It has always been under 5 miles per hour, but it has always been total stoppage, like someone hit me from behind. I think I figured out that the emergency braking system gets nervous when objects are close, and when I touched the brake with my foot the emergency braking system would freak out and slam the brakes. I talked to a person who had an electric Volvo who said the slamming of the brakes freaked him out, too. I finally went into the Settings and turned off the emergency braking system (but left on the warning.) I've not had the experience, since.

We leased this vehicle and within the first week of driving it we noticed that surface of the dashboard reflects in the windshield, severely impacting visibility. The problem is that the car has a grill which I believe is to vent air into the cabin. The design of it is very poor and the light reflects off it, severely impacting our ability to see other cars and pedestrians. In certain light conditions we do not feel safe to drive the car as. There have been multiple incidents that caused us to initiate an emergency brake because we could not see other vehicles. Upon researching online, there are hundreds of reports with thousands of people commenting. A company now even makes a dashboard mat to reduce the glare issue. I have not been able to take a good picture that adequately demonstrates the issue so I am attaching one from someone who was able to take a photo of what it looks like to the human eye. I am convinced that this situation warrants a recall.

Illinois I-94 Westbound near Torrence Ave. Exit 5:35/5:36AM 2/5/2025 cruise set at 70mph, passing 18 wheeler on my right and a car was passing me on my left. No car in front of or behind me in my lane of travel. Vehicle braked hard all by itself. I had to cancel the cruise control to stop the braking. This could have been bad had there actually been another vehicle directly behind me when this happened. It has happened before but I did not record the details. I will be making a written record of this failure and reporting it every time it happens.

The 2024-2025 Chevy Equinox EV has a major safety issue with overwhelming windshield reflections/glare caused by the odd design choices of the sculpted dash. The dash of the car has a slotted design unlike any I have seen in any other car, including other Chevy models currently available at Chevrolet dealerships. This slotted design creates numerous surfaces across the entire width of the dash that easily reflect the light thus producing a horrible glare on the windshield. I first noticed this issue within minutes of my first test driving a '24 model back in May 2024, something the salesperson had claimed to be unaware of but quickly conceded it was bad. As we changed direction the problem subsided and he felt it was probably only a rare issue. I finally took delivery of a '25 model on 01/16/25 & discovered that the issue persists more frequently than expected & is bad enough to block a clear view of what is in front of the vehicle. The problem is worse during bright, sunny days & from certain angles to the sun, but I have also noticed it on cloud covered days as well. It has been suggested that I use polarized glasses to mitigate the issue, & they do help, but they create other issues such as clearly seeing controls inside the car or viewing the rearview mirror as they darken everything. I am concerned that at some point, when the reflections are very bad, that I will not see a car backing out of a space in front of me or a pedestrian starting to walk out in front of me. It's unbelievable Chevy did not recognize this issue & address it at some point during all their testing before releasing this car for sale. I have found that this issue is a huge problem for many others & vehicle specific forums online are full of people having the same bad experience. I in fact acquired the link to this form from someone in those forums who was also having this issue. Something needs to be done by Chevy to fix this before someone is seriously hurt or worse.

My car which is FWD, not AWD, is exhibiting the same behavior that the recall for the AWD Equinox has. With Adaptive Cruise engaged the car does not slow down or brake when approaching vehicles in front of it. I have to intervene and apply the brakes to prevent hitting the vehicle in front. Also, with ACC on the vehicle will accelerate on its own. I have to intervene to slow the vehicle down. For example, I set the ACC to 68 mph, all of a sudden it starts speeding up. I had to intervene when it hit 73 mph as it approached vehicles in front of me without slowing down.

The car's dashboard has a patterned design (stripes or bars) which casts an extremely distracting glare onto the windshield. It is noticeable in all conditions, but in sunny conditions I have especially noticed this interferes with my ability to see the road and/or pedestrians. I had one incident where I was turning off a road and into a parking lot and nearly missed a pedestrian on the sidewalk because of this (in sunny weather). This is a well-known issue among Chevy Equinox EV owners, starting with the original 2024 model and continuing with the 2025 model. It is so much of an issue that third-party companies have started selling mats that sit on the dashboard to reduce the striped glare.
